摘要
目的 分析全面二孩政策实施前后上海市静安区医疗机构产妇的生育特征变化趋势,为上海市推进和完善三孩生育政策提供依据。方法 采用回顾性流行病学调查法,通过上海市静安区妇幼卫生信息登记表,收集2014年1月—2018年12月期间在上海市静安区各医疗助产机构生产的国内产妇相关信息。结果 2014—2018年上海市静安区二孩产妇平均比例为26.46%;全面二孩政策实施前后二孩产妇占比分别为23.28%和31.05%,差异有统计学意义(P<0.05);一孩和二孩产妇的平均年龄、文化程度为大专和本科的人员比例、剖宫产率在政策实施后均上升。年龄分层的分析结果显示,政策实施后<35岁的一孩产妇和≥35岁一孩、二孩产妇剖宫产率均上升,差异有统计学意义(P<0.05)。结论 上海市生育形势面临挑战,建议加大三孩政策宣传力度,提升妇幼卫生服务和管理质量,降低剖宫产率,保障母婴安全。
Objective To analyze the changing trend of the birth characteristics of mothers in medical institutions in Jing'an District of Shanghai before and after the implementation of the universal two-child policy,so as to provide basis for promoting and improving the three-child policy in Shanghai.Methods By the method of retrospective epidemiological survey,the data of mothers from maternal and child health information registration system of Jing'an District of Shanghai was collected from January 2014 to December 2018.Results From 2014 to 2018,the average proportion of two-child mothers in Jing'an District of Shanghai was 26.46%.Before and after the implementation of the universal two-child policy,the proportion of two-child mothers was 23.28%and 31.05%,respectively,with significant difference(P<0.05).According to the age stratification,the cesarean section rate of the younger one child mothers and all the elderly mothers increased,the difference was statistically significant(P<0.05).Conclusion The procreation situation in shanghai is faced with challenge.It is suggested to strengthen the propaganda of the three-child policy,improve the quality of maternal and child health services and management,reduce the rate of cesarean section and ensure the safety of mothers and infants.
