摘要
目的分析C反应蛋白(CRP)、血清淀粉样蛋白A(SAA)和CD64指数检测在儿童呼吸道感染疾病诊断中的应用价值。方法收集2020年1月至2021年4月安徽省儿童医院收治的因呼吸道感染住院的患儿82例,根据临床检查结果分为病毒感染组(50例)和细菌感染组(32例),选择同期30例眼科体检患儿作为对照组。比较三组CRP、SAA和CD64指数的差异,分析评价其对儿童呼吸道感染疾病的诊断价值。结果病毒感染组和细菌感染组血清CRP、SAA和CD64指数均明显高于对照组[(12.8±3.4)mg/L、(29.6±13.7)mg/L比(2.7±1.0)mg/L,(32.5±8.4)mg/L、(59.0±19.8)mg/L比(5.9±1.5)mg/L,(4.45±1.12)、(3.14±0.68)比(1.12±0.24)](P<0.01),且细菌感染组均高于病毒感染组(P<0.01)。病毒感染组和细菌感染组CRP、SAA及CD64指数单独及联合检测的阳性率均明显高于对照组(P<0.05),且细菌感染组均高于病毒感染组(P<0.05)。CRP检测诊断细菌感染的灵敏度为78.12%、特异度为78.00%、准确度为78.05%,SAA检测的灵敏度为90.62%、特异度为68.00%、准确度为76.83%,CD64指数检测的灵敏度为43.75%、特异度为84.00%、准确度为68.30%,联合检测的灵敏度为96.88%、特异度为74.00%、准确度为98.78%。结论CRP、SAA、CD64指数在细菌所致的呼吸道感染中显著高于病毒感染,CRP、SAA、CD64指数在细菌感染中的诊断价值较高,且三者联合检测的诊断效能更佳。
Objective To analyze the applicative value of C-reactive protein(CRP),serum amyloid A(SAA)and CD64 index in the diagnosis of pediatric respiratory tract infections.Methods A total of 82 children hospitalized for respiratory tract infection in Anhui Provincial Children′s Hospital from Jan.2020 to Apr.2021 were included.According to the clinical examination results,they were divided into a viral infection group(50 cases)and a bacterial infection group(32 cases).30 children for eye examination during the same period were included as a control group.The differences in the CRP,SAA,and CD64 between the three groups were compared to analyze and evaluate their diagnostic value for pediatric respiratory tract infectious diseases.Results The serum CRP,SAA and CD64 index of the viral infection group and the bacterial infection group were significantly higher than those of the control group[(12.8±3.4)mg/L and(29.6±13.7)mg/L vs(2.7±1.0)mg/L,(32.5±8.4)mg/L and(59.0±19.8)mg/L vs(5.9±1.5)mg/L,(4.45±1.12)and(3.14±0.68)vs(1.12±0.24)](P<0.01),and the bacterial infection group were higher than the viral infection group(P<0.01).The positive rates of CRP,SAA and CD64 index alone or in combination of the viral infection group and the bacterial infection group were significantly higher than those of the control group(P<0.05),and those in the bacterial infection group were all higher than those in the viral infection group(P<0.05).The sensitivity,specificity and accuracy of CRP detection in the diagnosis of bacterial infection was 78.12%,78.00%and 78.05%respectively,of SAA detection was 90.62%,68.00%and 76.83%,respectively,of CD64 index detection was 43.75%,84.00%and 68.30%respectively,and of the combined detection was 96.88%,74.00%and 98.78%respectively.Conclusion CRP,SAA and CD64 indexes are significantly higher in the bacterial respiratory tract infections than in the viral respiratory tract infections,so they have higher diagnostic value for bacterial infections,and the combined tests of the three indexes have a better diagnostic efficiency.
