摘要
目的 探讨骨髓增生异常综合征患者感染的影响因素.方法 回顾性分析南阳市第二人民医院2020年1月-2022年11月收治的248例骨髓增生异常综合征患者的临床资料,根据感染情况分为感染组(n=59)和未感染组(n=189),采用多因素Logistic回归分析骨髓增生异常综合征患者感染的影响因素,并构建回归方程,采用受试者工作特征(ROC)曲线分析回归方程对骨髓增生异常综合征患者感染的预测价值.结果 多因素Logistic回归分析结果显示,合并糖尿病、使用糖皮质激素、使用地西他滨是骨髓增生异常综合征患者感染的危险因素(OR=4.309、2.777、4.959,P<0.05),血清白蛋白、使用粒细胞集落刺激因子是骨髓增生异常综合征患者感染的保护因素(OR=0.343、0.878,P<0.05);ROC曲线分析结果显示,回归方程预测骨髓增生异常综合征患者感染的曲线下面积(AUC)值为0.824,敏感度为77.97%,特异度为71.96%.结论 根据骨髓增生异常综合征患者感染影响因素构建相关的回归方程预测价值较好,临床可采用此回归方程对骨髓增生异常综合征患者感染的风险进行预测.
OBJECTIVE To investigate the influencing factors of infection in patients with myelodysplastic syndrome.METHODS The clinical data of 248 patients with myelodysplastic syndrome admitted to Nanyang Second People's Hospital from Jan.2020 to Nov.2022 were retrospectively analyzed,and the patients were divided into an infected group(59 cases)and an uninfected group(189 cases)according to whether they were infected.Multivariate Logistic regression was used to analyze the risk factors of infection in patients with myelodysplastic syndrome,and relevant regression equations were constructed.The predictive value of regression equations for infection in patients with myelodysplastic syndrome was analyzed by receiver operating characteristic curve(ROC).RESULTS Multivariate logistic regression analysis showed that combined diabetes mellitus,use of glucocorticoid and decitabine were risk factors for infection in patients with myelodysplastic syndrome(OR=4.309.2.777.4.959,P<O.05).Higher level of serum albumin and use of granulocyte colony-stimulating factor were protective factors for infection in patients with myelodysplastic syndrome(OR=0.343,0.878,P<0.05).ROC curve analysis showed that the area under the curve(AUC)value of the regression equation for predicting infection in patients with myelodysplastic syndrome was 0.824,with a sensitivity of 77.97%,and a specificity of 71.96%.CONCLUSION The regression equation based on the factors influencing infection in patients with myelodysplastic syndrome had good predictive value,which could be used to predict the risk of infection in patients with myelodysplastic syndrome.
