摘要
目的 探讨无创呼吸机序贯治疗配合早期肺康复治疗在慢性阻塞性肺疾病(chronic obstructive pulmonary disease, COPD)并呼吸衰竭患者中的效果。方法 选取2022年1月—2023年3月北京丰台医院收治的COPD并呼吸衰竭患者66例,按照随机数表法分为对照组(n=33)和观察组(n=33)。对照组采用无创呼吸机序贯治疗,观察组联合早期肺康复治疗。比较两组干预效果,肺功能指标及生活质量。结果 观察组总有效率为93.94%,高于对照组的75.76%,差异有统计学意义(χ^(2)=4.243,P<0.05)。干预后,观察组用力肺活量、第1秒用力呼气容积、呼吸峰值流速水平均高于对照组,生活质量量表中呼吸症状、活动能力、疾病影响维度评分均低于对照组,差异有统计学意义(P<0.05)。结论 无创呼吸机序贯治疗配合早期肺康复治疗COPD并呼吸衰竭能有效改善患者肺功能,提高生活质量,临床效果显著。
Objective To investigate the effect of sequential noninvasive ventilator therapy combined with early pulmonary rehabilitation in patients with chronic obstructive pulmonary disease(COPD)and respiratory failure.Methods A total of 66 patients with COPD and respiratory failure admitted to Beijing Fengtai Hospital from January 2022 to March 2023 were selected and divided into control group(n=33)and observation group(n=33)according to random number table method.The control group received sequential treatment with noninvasive ventilator,and the observation group combined with early pulmonary rehabilitation.The intervention effect,lung function index and quality of life were compared between the two groups.Results The total effective rate of the observation group was 93.94%,higher than that of the control group(75.76%),and the difference was statistically significant(χ^(2)=4.243,P<0.05).After intervention,the forced vital capacity,forced expiratory volume at the first second and peak respiratory flow rate in the observation group were higher than those in the control group,and the scores of respiratory symptoms,activity ability and disease impact dimension in the quality of life scale were lower than those in the control group,the difference was statistically significant(P<0.05).Conclusion Noninvasive ventilator sequential therapy with early pulmonary rehabilitation treatment of COPD with respiratory failure can effectively improve patients'pulmonary function and quality of life,and the clinical effect is remarkable.
