摘要
目的分析腰椎间盘突出症(lumbar disc herniation,LDH)非手术治疗住院费用的中、西医疗法差异,为控制LDH住院费用、降低患者负担、推动LDH治疗方法发展提供参考依据。方法通过某市医保数据库,收集2019—2020年4094例LDH患者病案首页信息和明细费用数据,采用单因素分析、多元线性回归分析等方法探讨患者中、西医疗法住院费用差异。结果中医组LDH患者住院费用中位数为7662.16元,高于西医组(6658.33元)(P<0.001)。控制性别、年龄等因素后,西医治疗费用低于中医。结论中医组住院费用、住院天数高于西医组,建议推行中医LDH临床路径,降低住院费用。中医组患者自费费用低于西医组,应积极探索LDH中医按病种付费方式,促进中医药传承创新发展。
Objective To analyze the differences in the cost of hospitalization between traditional Chinese medicine(TCM)therapy and western therapy for non-surgical treatment of lumbar disc herniation(LDH),so as to provide reference for controlling the cost of LDH hospitalization,reducing the burden of patients and promoting the development of LDH treatment.Methods Through the medical insurance database of a city,the medical record home page information and detailed cost data of 4094 LDH patients from 2019 to 2020 were collected.The single factor analysis and multivariable linear regression analysis were used to explore the hospitalization costs of patients with TCM and Western medicine.Results The median hospitalization cost of LDH patients in TCM group was 7662.16 yuan,which was higher than the 6658.33 yuan in western medicine group(P<0.001).After controlling gender,age and other factors,western medicine treatment costs were lower than TCM.Conclusion The hospitalization costs and length of hospital stay in the TCM group were higher than those in the western medicine group.It is suggested to implement the clinical pathway of LDH in TCM to reduce the hospitalization expenses.The self-paid expenses of patients in the TCM group were lower than those in the western medicine group.We should actively explore the payment method of LDH TCM by disease and promote the inheritance and innovation of TCM.
