摘要
目的分析术中回收式自体输血对前置胎盘剖宫产术中患者凝血因子及免疫功能的影响。方法选取2020年2月至2022年3月我院收治的82例前置胎盘行剖宫产术患者为研究对象,根据术中输血方式将其分为对照组和观察组,各41例。对照组采取异体输血,观察组采取术中回收式自体输血。比较两组的凝血因子水平、免疫功能、血常规指标及输血不良反应发生情况。结果术后7 d,观察组的凝血因子Ⅷ、Ⅸ、Ⅹ、ⅩⅢ水平高于对照组(P<0.05)。术后7 d,观察组的白细胞分化抗原4阳性(CD4^(+))、白细胞分化抗原4阳性/白细胞分化抗原8阳性(CD4^(+)/CD8^(+))高于对照组,CD8^(+)低于对照组(P<0.05)。术后7 d,两组的血小板计数(PLT)、红细胞比容(Hct)、红细胞计数(RBC)、血红蛋白(Hb)水平比较,差异无统计学意义(P>0.05)。观察组的输血不良反应总发生率低于对照组(P<0.05)。结论术中回收式自体输血相较于异体输血方式,不仅能够有效调节前置胎盘剖宫产术中患者凝血因子水平,改善免疫功能及血常规指标,且输血不良反应较少,值得推广。
Objective To analyze the effects of intraoperative salvaged autologous blood transfusion on coagulation factors and immune function in patients with placenta previa during cesarean section.Methods A total of 82 patients with placenta previa who underwent cesarean section in our hospital from February 2020 to March 2022 were selected as the research objects.According to the intraoperative blood transfusion method,the patients were divided into control group and observation group,with 41 cases in each group.The control group received allogeneic blood transfusion,and the observation group received intraoperative salvaged autologous blood transfusion.The levels of coagulation factors,immune function,blood routine indexes and adverse reactions of blood transfusion were compared between the two groups.Results At 7 d after operation,the levels of coagulation factorsⅧ,Ⅸ,Ⅹ,ⅩⅢin the observation group were higher than those in the control group(P<0.05).At 7 d after operation,the cluster of differentiation 4 positive(CD4^(+))and cluster of differentiation 4 positive/cluster of differentiation 8 positive(CD4^(+)/CD8^(+))in the observation group were higher than those in the control group,CD8^(+)was lower than that in the control group(P<0.05).At 7 d after operation,there were no significant differences in the levels of platelet count(PLT),hematocrit(Hct),red blood cells(RBC)and hemoglobin(Hb)between the two groups(P>0.05).The total incidence of adverse reactions of blood transfusion in the observation group was lower than that in the control group(P<0.05).Conclusion Compared with allogeneic blood transfusion,intraoperative salvaged autologous blood transfusion can not only effectively regulate the level of coagulation factors in patients with placenta previa during cesarean section,improve immune function and blood routine indexes,but also have fewer adverse reactions of blood transfusion,which is worthy of promotion.
