摘要
目的探讨男性肥胖患者接受腹腔镜下袖状胃切除术(LSG)的临床疗效及其对男性肥胖患者激素水平的影响。方法收集2018年1月至2021年6月在新疆维吾尔自治区人民医院行LSG的男性肥胖患者31例,根据是否合并糖尿病将患者分为糖尿病组(7例)和非糖尿病组(24例)。比较2组患者术前及术后6个月的总睾酮、雌二醇、促黄体生成素和卵泡刺激素水平,观察全部患者的减重效果以及减重手术对血糖及激素水平的影响。结果全部患者术后体质量、体重指数和腰围均低于术前[(92±11)kg比(129±21)kg、(31±4)kg/m^(2)比(43±7)kg/m^(2)、(105±9)cm比(134±16)cm],差异均有统计学意义(均P<0.001)。全部患者术后空腹血糖、空腹胰岛素、雌二醇和促黄体生成素水平均低于术前,总睾酮和卵泡刺激素水平均高于术前(均P<0.05)。结论LSG可以显著降低男性肥胖患者体质量和血糖,并且有利于调整体内性激素水平。
Objective To investigate the clinical efficacy of laparoscopic sleeve gastrectomy(LSG)in male obese patients and its effect on hormone levels.Methods A total of 31 male obese patients who underwent LSG in People′s Hospital of Xinjiang Uygur Autonomous Region from January 2018 to June 2021 were collected.The patients were divided into diabetes mellitus group(7 cases)and non-diabetes mellitus group(24 cases)according to whether they had diabetes mellitus.The levels of total testosterone,estradiol,luteinizing hormone and follicle stimulating hormone were compared between the two groups before and 6 months after surgery.The weight loss effect of all patients and the effect of weight loss surgery on blood glucose and hormone levels were observed.Results The postoperative body mass,body mass index and waist circumference of the patients were lower than those before surgery[(92±11)kg vs(129±21)kg,(31±4)kg/m^(2) vs(43±7)kg/m^(2),(105±9)cm vs(134±16)cm](all P<0.001).The levels of fasting blood glucose,fasting insulin,estradiol and luteinizing hormone in the patients after surgery were lower than those before surgery,and the levels of total testosterone and follicle stimulating hormone were higher than those before surgery(all P<0.05).Conclusions LSG can significantly reduce body mass,blood glucose of male obese patients,and help to adjust the level of sex hormones.
