摘要
目的 研究自身免疫性溶血性贫血(自免溶贫)对于血型鉴定、交叉配血的临床影响。方法 选取2018年1月-2022年12月在本院进行配血的患者共364例为本次研究的试验对象,均进行抗人球蛋白试验、血型鉴定及交叉配血试验,统计自免溶贫发生率,分析自免溶贫患者的直接抗人球蛋白试验(DAT)及间接抗人球蛋白试验(IAT)强度情况,计算自免溶贫患者的抗体类型和不同DAT强度、不同抗体类型的ABO血型正反定型相符率、交叉配血相合率。结果 本组364例配血患者中自免溶贫患者38例,发生率10.44%。38例自免溶贫患者的DAT与IAT试验结果均为阳性,强度均集中在++~+++之间;抗体类型以温抗体为主。在DAT试验阳性程度不断上升的情况下,患者ABO血型正反定型相符率随之下降,且交叉配血相合率也有显著降低,数据差异有统计学意义(P<0.05)。不同抗体类型患者的ABO血型正反定型相符率、交叉配血相合率数据差异无统计学意义(P>0.05)。结论 自免溶贫患者的常见抗体为温抗体,且DAT强度同交叉配血相合与否、血型相符与否有一定相关性,在发生交叉配血不相合及血型不相符的情况时需要通过吸收放散试验进行改善,以保证效果。
Objective To study clinical effect of autoimmune hemolytic anemia(AIHA)on blood type identification and cross matching.Methods The paper chose 364 patients with blood matching from January 2018 to December 2022 for study,calculated incidence of autoimmune hemolysis after anti human globulin testing,blood group identification and cross matching test,analyzed direct anti human globulin test(DAT)and indirect anti human globulin test(IAT)intensity of autoimmune hemolysis patients,calculated antibody types and different DAT intensities of autoimmune hemolysis patients,positive and negative typing matching rate and cross matching matching rate of ABO blood types with different antibody types.Results Among 364 blood matching patients in the group,there were 38 patients with autoimmune hemolytic anemia,with an incidence rate of 10.44%.DAT and IAT test results of 38 patients with autoimmune hemolytic anemia were all positive,with intensities concentrated between++and++.Main types of antibodies were warm antibodies.As positive degree of DAT test increased,matching rate of positive and negative typing of ABO blood group decreased,cross matching rate significantly decreased,with statistical significance(P<0.05).There was no statistically significant difference in the ABO blood group positive and negative typing matching rate and cross matching rate data among patients with different antibody types(P>0.05).Conclusion Common antibodies of patients with autoimmune hemolytic anemia are warm ones,there is some certain correlation between compatibility of DAT intensity and cross matching,blood type matching.Under condition of mismatched cross matching and blood type,it is necessary to improve it through absorption and diffusion test to ensure effectiveness.
