摘要
目的分析子宫内膜息肉电切术后患者妊娠结局及其影响因素。方法回顾性分析2016年1月至2017年1月于锦州医科大学沈阳市妇婴医院因子宫内膜息肉(EP)行子宫内膜息肉电切术后120例妊娠患者的临床资料,采用单因素和多因素Logistic回归分析子宫内膜息肉电切术后异常妊娠的影响因素。结果120例子宫内膜息肉电切术后患者中正常妊娠并分娩68例,正常妊娠率为56.7%,流产、早产等异常妊娠52例,异常妊娠率为43.33%。子宫内膜息肉电切术后正常妊娠患者与子宫内膜息肉电切术后异常妊娠患者年龄、手术时间、扩张直径、息肉个数、术后妊娠间隔比较差异无统计学意义;子宫内膜息肉电切术术后正常妊娠患者与子宫内膜息肉电切术术后异常妊娠患者体重指数(BMI)、手术次数、流产次数、息肉位置比较差异有统计学意义(P<0.05)。多因素Logistic回归分析结果显示,流产次数≥2次是影响子宫内膜息肉电切术术后患者妊娠结局的独立危险因素(P<0.05)。结论流产次数≥2次是影响子宫内膜息肉电切术术后患者妊娠结局的独立危险因素,临床医生应准确评估妊娠相关危险因素,减少宫腔手术操作,避免不必要的医源性手术损伤,指导患者适当控制BMI,可有效提高临床妊娠率,改善妊娠结局。
Objective The pregnancy outcome and its influencing factors were analyzed after trans-cervical resection of polyps.Methods The clinical data of 120 patients with pregnancy who underwent hysteroscopic electroresection for endometrial polypectomy in ShenyangWomen's and Children's Hospital of Jinzhou Medical University from January 2016 to January 2017 were retrospectively analyzed,univariate and multivariate Logistic regression analysis were used to analyze the related factors that may affect pregnancy after hysteroscopic electrotomy.Results Among the 120 patients with endometrial polypectomy,68 cases had normal pregnancy and delivery,the normal pregnancy rate was 56.7%,and 52 cases had abnormal pregnancy such as abortion and preterm birth,the abnormal pregnancy rate was 43.33%.There was no significant difference in age,operation time,dilation diameter,number of polyps and postoperative pregnancy interval between normal and abnormal pregnancy patients after endometrial polypectomy;there were sgnificant difference in the body mass index(BMI),operation times and abortion times,polyp location between normal pregnancy patients after endometrial polypectomy and abnormal pregnancy patients after endometrial polypectomy(P<0.05).Multivariate Logistic regression analysis showed that the number of abortion≥2 times was an independent risk factor for pregnancy outcomes in patients with endometrial polypectomy after surgery(P<0.05).Conclusion The number of abortion≥2 times is an independent risk factor affecting the pregnancy outcome of patients after electroresection of endometrial polyps.Clinicians should accurately evaluate pregnancy-related risk factors,reduce uterine surgery,avoid unnecessary iatrogenic surgical injury,and guide patients to properly control BMI,which can effectively improve the clinical pregnancy rate and improve pregnancy outcome.
