摘要
目的:探讨白及粉外用在肛瘘术后创面修复的临床效果及对血清炎症因子的影响。方法:将2021年1月到2022年6月我院收治的80例肛瘘手术患者随机分为观察组与对照组(各40例)。对照组术后采取常规凡士林覆油纱盖创面,观察组术后在此基础上增加白及粉外敷,对比两组患者创面修复的临床疗效,术后瘢痕情况,肛门控便能力以及炎性因子表达水平。结果:观察组治疗总有效率(95.00%)比对照组高(75.00%)(P<0.05);治疗7 d和14 d后,观察组患者创面分泌物、创面肉芽生长评分、疼痛评分、创面愈合时间均低于对照组(P<0.05),两组患者术后6个月控便能力均分明显低于术后1个月,但观察组与对照组之间差异无统计学意义(P>0.05),术后4、6、8周,观察组瘢痕情况评分均低于对照组(P<0.05);治疗后,观察组IL-6、IL-1、TNF-α相关炎症因子水平比对照组低(P<0.05)。结论:在肛瘘术后运用白及粉外敷可提升治疗效果,改善创面修复及疼痛情况,降低炎性水平,并减轻患者术后瘢痕情况,对于肛门控便能力无不良影响,值得临床应用推广。
Objective:To investigate the clinical effect of topical application of Bletilla striata Powder in wound repair after anal fistula operation and its influence on serum inflammatory factors.Methods:80 patients with anal fistula surgery admitted to our hospital from January 2021 to June 2022 were randomly divided into an observation group and a control group(with 40 cases in each).The control group was treated with conventional vaseline oil gauze to cover the wound after surgery,and the observation group was treated with Bletilla striata Powder on the basis of external application.The clinical efficacy of wound repair,postoperative scar condition,anal stool control ability and expression level of inflammatory factors were compared between the two groups.Re⁃sults:The total effective rate of the observation group(95.00%)was higher than that of the control group(75.00%)(P<0.05).After 7 and 14 days of treatment,wound secretions,wound granulation growth scores,pain scores and wound healing time in the observation group were lower(P<0.05).The average score of stool control ability in the two groups was significantly lower than that in the one month after surgery,but there was no differ⁃ence between the observation group and the control group(P>0.05).4,6,8 weeks after surgery,Scar scores in observation group were lower than those in the control group(P<0.05).After the treatment,the levels of IL-6,IL-1 and TNF-αrelated inflammatory factors in observation group were lower(P<0.05).Conclusion:External application of Bletilla striata powder after anal fistula surgery can improve the therapeutic effect,wound repair and pain,reduce inflammatory levels,and alleviate postoperative scar of patients.It has no adverse effect on the anal stool control ability,and is worthy of clinical application and promotion.
