重复经颅磁刺激在难治性抑郁症患者中的疗效及对神经内分泌功能的影响 被引量:5

Effect of Repetitive Transcranial Magnetic Stimulation in Patients with Refractory Depression and Influence for Neuroendocrine Function

摘要 目的:探究重复经颅磁刺激在难治性抑郁症患者中的疗效及对神经内分泌功能的影响。方法:将2019年10月—2022年3月泉州市第三医院的80例难治性抑郁症患者根据随机数字表法分为两组,每组40例。对照组进行综合治疗,观察组则在对照组的基础上进行重复经颅磁刺激治疗。比较两组治疗总有效率、不良反应发生率、治疗前后的汉密尔顿抑郁量表(HAMD)评分结果及神经内分泌指标[皮质醇(Cor)、促肾上腺皮质激素(ACTH)、促甲状腺激素(TSH)、游离三碘甲状腺原氨酸(FT_(3))及游离四碘甲状腺原氨酸(FT_(4))]。结果:观察组治疗总有效率显著高于对照组(P<0.05),两组不良反应发生率差异无统计学意义(P>0.05),治疗前两组的HAMD评分结果及神经内分泌指标比较,差异均无统计学意义(P>0.05),治疗4、8周后两组的HAMD评分结果及神经内分泌指标均显著优于治疗前,且观察组上述方面均显著优于对照组(P<0.05)。结论:重复经颅磁刺激在难治性抑郁症患者中的疗效较好,对于患者的神经内分泌功能具有较好的改善作用。 Objective:To investigate the effect of repetitive transcranial magnetic stimulation in patients with refractory depression and influence for neuroendocrine function.Method:A total of 80 patients with refractory depression in the Third Hospital of Quanzhou from October 2019 to March 2022 were divided into two groups,with 40 cases in each group by the method of random number table.The control group was treated with comprehensive treatment,the observation group was treated with repetitive transcranial magnetic stimulation therapy on the basis of the control group.Then the total effective rates,the adverse reaction rates,Hamilton depression scale(HAMD)score and neuroendocrine indexes[cortisol(Cor),adrenocorticotropic hormone(ACTH),thyroid stimulating hormone(TSH),free triiodothyronine(FT_(3))and free tetraiodothyronine(FT_(4))]before and after treatment of two groups were compared.Result:The total effective rate of the observation group was significantly higher than that of the control group(P<0.05),the adverse reaction rates of two groups were not statistically significant(P>0.05),the HAMD scores and neuroendocrine indexes of two groups before treatment were not statistically significant(P>0.05),the HAMD scores and neuroendocrine indexes of two groups at 4 and 8 weeks after the treatment were significantly better than those before treatment,and those items of observation group were all significantly better than those of control group(P<0.05).Conclusion:The effect of repetitive transcranial magnetic stimulation in patients with refractory depression is better,and it can significantly improve the neuroendocrine function of patients.
