摘要
目的探究经阴道三维子宫输卵管超声造影诊断先天性子宫畸形的临床价值。方法选取2019年1月至2021年4月于本院接受检查的183例高度疑似先天性子宫畸形患者作为研究对象,均进行经阴道二维子宫输卵管超声造影和经阴道三维子宫输卵管超声造影,以腹(宫)腔镜诊断为金标准,比较经阴道三维子宫输卵管超声造影诊断与经阴道二维子宫输卵管超声造影诊断先天性子宫畸形的检出率及诊断效能。结果183例高度疑似先天性子宫畸形患者确诊180例,经阴道三维子宫输卵管超声造影诊断检出率为95.56%,高于经阴道二维子宫输卵管超声造影诊断的83.33%,差异有统计学意义(P<0.05)。183例高度疑似先天性子宫畸形患者中,经阴道二维子宫输卵管超声造影诊断确诊150例,经阴道三维子宫输卵管超声造影诊断确诊172例。经阴道三维子宫输卵管超声造影诊断的准确度和灵敏度均高于经阴道二维子宫输卵管超声造影诊断,差异有统计学意义(P<0.05);两种检测方法特异度比较差异无统计学意义。结论在先天性子宫畸形的诊断中,与经阴道二维子宫输卵管超声造影相比,经阴道三维子宫输卵管超声造影具有较高的检出率和准确率,可作为子宫畸形的首选诊断方法,具有较高的应用价值和推广意义。
Objective To investigate the clinical value of transvaginal three-dimensional hysterotubal contrast-enhanced ultrasound in the diagnosis of congenital uterine malformations.Methods A total of 183 patients with high suspicion of congenital uterine malformation who were examined in our hospital from January 2019 to April 2021 were selected as the research subjects.All of them underwent transvaginal two-dimensional hysterosalpinx contrast-enhanced ultrasound and transvaginal three-dimensional hysterosalpinx contrast-enhanced ultrasound,with abdominal(uterine)endoscopy as the gold standard for diagnosis,to compare the detection rate and diagnostic efficacy of transvaginal three-dimensional hysterotubal contrast-enhanced ultrasound and transvaginal two-dimensional hysterotubal contrast-enhanced ultrasound in the diagnosis of congenital uterine malformations.Results 180 of 183 patients with highly suspected congenital uterine malformation were confirmed.The detection rate of transvaginal three-dimensional hysterosalpinx contrast-enhanced ultrasound was 95.56%,which was higher than 83.33%of transvaginal two-dimensional hysterosalpinx contrast-enhanced ultrasound,the difference was statistically significant(P<0.05).Among the 183 patients with highly suspected congenital uterine malformation,150 cases were confirmed by transvaginal two-dimensional vaginal hysterotubal contrast-enhanced ultrasound diagnosis,and 172 cases were confirmed by transvaginal three-dimensional vaginal hysterotubal contrast-enhanced ultrasound diagnosis.The accuracy and sensitivity of transvaginal three-dimensional hysterosalpinx ultrasound diagnosis were higher than that of transvaginal two-dimensional hysterosalpinx ultrasound diagnosis,and the difference was statistically significant(P<0.05).There was no significant difference in specificity between the two methods.Conclusion In the diagnosis of congenital uterine malformations,compared with transvaginal two-dimensional hysterosalpinx contrast-enhanced ultrasound,transvaginal three-dimensional hysterosalpinx contrast-enhanced ultrasound has a higher detection rate and accuracy,and can be used as the first choice for the diagnosis of uterine malformations,and has a higher application value and promotion significance.
