摘要
目的探讨阴道三维超声OmniView成像应用于先天性子宫畸形鉴别和分型诊断中的价值。方法将盘锦辽油宝石花医院2019-01-03-2022-01-03收治的88例疑似先天性子宫畸形患者纳为研究对象,均行阴道三维及二维超声检查,在OmniView成像中行宫腔形态、子宫外形及宫颈管的观察,以临床结果作为金标准,观察2种检查对子宫畸形类型的检出情况及分型诊断效果。结果88例疑似先天性子宫畸形患者经临床检查结果确诊83例,三维超声检查灵敏度(97.59%)高于二维超声(86.75%),差异有统计学意义,t=6.760,P=0.009;三维超声检查特异度(80.00%)、阳性预测值(98.78%)及阴性预测值(66.67%)与二维超声(80.00%、98.63%、26.67%)检查比较,差异无统计学意义,χ^(2)值分别为0.000、0.007和1.459,P值分别为1.000、0.934和0.227。分型诊断结果显示,宫腔镜手术确诊48例(弓形子宫4例,单角子宫3例,不完全纵膈28例,完全纵膈子宫13例);腹腔镜+宫腔镜手术确诊13例(均为双子宫),核磁共振检查确诊6例(单角子宫1例,双子宫5例);输卵管碘油造影确诊16例(弓形子宫5例,不完全纵膈9例,双子宫2例);三维超声诊断符合率(98.80%)高于二维超声(87.95%),差异有统计学意义,χ^(2)=7.886,P=0.005。结论阴道三维超声OmniView成像应用于先天性子宫畸形鉴别和分型诊断中符合率及诊断价值更高。
Objective To investigate the value of transvaginal three-dimensional ultrasound OmniView imaging in the differential diagnosis and typing of congenital uterine malformations.Methods A total of 88 patients with suspected congenital uterine malformations admitted to Panjin Liaoyou Baoshihua Hospital from January 3,2019 to January 3,2022 were included in the study.All of them underwent vaginal three-dimensional and two-dimensional ultrasound examinations.The morphology of the uterine cavity,the shape of the uterus and the cervical canal were observed in OmniView imaging.The clinical results were used as the gold standard to observe the detection of the types of uterine malformations and the diagnostic effect of the two examinations.Results Eighty-three of 88 patients with suspected congenital uterine malformations were confirmed by clinical examination.The sensitivity of three-dimensional ultrasound(97.59%)was higher than that of two-dimensional ultrasound(86.75%),with a statistically significant difference(t=6.760,P=0.009).The specificity of three-dimensional ultrasound was 80.00%,the positive predictive value was 98.78%,and the negative predictive value was 66.67%,there was no significant difference compared with 80.00%,98.63%,26.67%of two-dimensional ultrasound,χ^(2)values were 0.000,0.007 and 1.459,respectively,and P values were 1.000,0.934 and 0.227,respectively.The results of classification diagnosis showed that 48 cases were confirmed by hysteroscopy(4 cases of arcuate uterus,3 cases of uniangular uterus,28 cases of incomplete mediastinum,13 cases of complete mediastinum),13 cases were confirmed by laparoscopy+hysteroscopy(all of them were double uterus),6 cases were confirmed by MRI(1 case of single horn uterus,5 cases of double uterus),16 cases were diagnosed by hysterosalpingography(5 cases with arcuate uterus,9 cases with incomplete mediastinum,2 cases with double uterus).The coincidence rate of three-dimensional ultrasound(98.80%)was higher than that of two-dimensional ultrasound(87.95%),the difference was statistically significant,χ^(2)=7.886,P=0.005.Conclusion The coincidence rate and diagnostic value of vaginal three-dimensional ultrasound OmniView imaging in the differentiation and classification of congenital uterine malformations are higher.
