摘要
目的探讨采用基于“内热法理论”内热针治疗寒湿型腰椎间盘突出症(LIDH)患者的临床疗效。方法选取2018年4月至2021年6月本院收治的40例寒湿型腰椎间盘突出症患者作为研究对象,按照随机数字表法分为内热针组和温针灸组,每组20例。内热针组采用内热针治疗,温针灸组采用常规温针灸治疗,比较两组临床疗效、治疗前后腰椎功能及疼痛情况[视觉模拟评分法(VAS)评分、腰椎功能(JOA)评分]及中医腰痛症状体征积分和寒湿证候积分。结果治疗后,内热针组治疗总有效率为95.00%,高于温针灸组的65.00%,差异有统计学意义(P<0.05)。治疗前,两组JOA、VAS评分比较差异无统计学意义;治疗后,两组JOA评分均高于治疗前,VAS评分均低于治疗前,且内热针组JOA评分高于温针灸组,VAS评分低于温针灸组,差异有统计学意义(P<0.05)。治疗前,两组中医腰痛症状体征积分和寒湿证候积分比较差异无统计学意义;治疗后,两组中医腰痛症状体征积分和寒湿证候积分均低于治疗前,且内热针组低于温针灸组,差异有统计学意义(P<0.05)。结论内热针治疗寒湿型LIDH疗效明显,可改善腰椎功能,缓解腰痛,有效改善寒湿型LIDH患者的临床症状,促进患者早日康复,值得临床推广应用。
Objective To investigate the clinical effect of internal heat needle based on"Internal Heat method theory"in the treatment of cold and wet lumbar disc herniation(LIDH).Methods 40 patients with cold-dampness lumbar disc herniation admitted to our hospital from April 2018 to June 2021 were selected as the research subjects,they were divided into the internal heat acupuncture group and the warm acupuncture group according to random number table method,with 20 cases in each group.The internal heat acupuncture group was treated with internal heat needle,and the warm acupuncture group was treated with conventional warm acupuncture,the clinical efficacy,lumbar function and pain(visual analog scale[VAS]score,lumbar function[JOA]score),symptoms and signs scores of Traditional Chinese Medicine lumbago and cold and dampness syndrome scores before and after treatment were compared between the two groups.Results After treatment,the total effective rate of internal heat acupuncture group was 95.00%,higher than 65.00%of warm acupuncture group,the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in JOA and VAS scores between the two groups;after treatment,the JOA scores of the two groups were higher than those before treatment,and the VAS scores were lower than those before treatment,and the JOA score in the internal heat acupuncture group was higher than that in the warm acupuncture group,and the VAS score was lower than that in the warm acupuncture group,the differences were statistically significant(P<0.05).Before treatment,there was no significant difference in the scores of symptoms and signs of lumbago and the scores of cold and dampness syndrome between the two groups;after treatment,the scores of symptoms and signs of lumbago and cold and dampness syndrome of the two groups were lower than before treatment,and the internal heat acupuncture group was lower than the warm acupuncture group,the differences were statistically significant(P<0.05).Conclusion Internal heat acupuncture is effective in the treatment of cold and wet LIDH,which can improve the function of lumbar spine,relieve lumbago,effectively improve the clinical symptoms of patients with cold and wet LIDH,and promote the early recovery of patients,which is worthy of clinical promotion and application.
