不同机种飞行员静息态脑血流量、结构和功能的差异研究 被引量:1

Study of the Differences in Cerebral Blood Flow,Structure and Function in the Resting State Among Pilots of Different Types of Aircrafts

摘要 目的比较空军不同机种(歼击机与运输机)飞行员和普通对照人群静息态脑血流量(cerebral blood flow,CBF)、结构和功能差异,为飞行员选拔提供理论依据。方法选取14名现役飞行员(歼击机飞行员8名,运输机飞行员6名)和9名普通对照人群(对照组)进行静息态下动脉自旋标记(arterial spin labeling,ASL)、弥散张量成像(diffusion tensor imaging,DTI)和静息态功能磁共振成像(resting-state functional magnetic resonance imaging,rs-fMRI)检查,对飞行员和对照组人群以及不同机种飞行员之间的差异脑区进行准确定位。结果ASL结果显示:飞行员左背外侧前额叶静息态CBF值高于对照组人群(P<0.01),歼击机飞行员左背外侧前额叶静息态CBF值高于运输机飞行员(P<0.01);DTI结果显示:飞行员大脑胼胝体和右放射冠的各向异性值(fractional anisotropy,FA)高于对照组人群(P<0.01),且歼击机飞行员大脑胼胝体FA高于运输机飞行员(P<0.01);rs-fMRI结果显示:飞行员静息态左右半球间的枕叶、颞下回和中央旁回基于体素的镜像同伦功能连接(voxel-mirrored homotopic connectivity,VMHC)值均高于对照组人群(P<0.01),且歼击机飞行员静息态左右半球间的小脑、颞中回和脑岛VMHC值均高于运输机飞行员(P<0.05)。结论与普通对照人群比较,经过长期的飞行训练,飞行员大脑左背外侧前额叶静息态CBF增加,大脑白质完整性增强和静息态脑网络连接增强。由于长期不同的飞行科目训练,相比运输机飞行员,歼击机飞行员大脑左背外侧前额叶静息态CBF增加,大脑白质完整性增强和静息态脑网络连接增强。 Objective To compare differences in cerebral blood flow(CBF),structure and function in the resting state among air force pilots of different types of aircrafts(fighter and transport)and ordinary control populations,so as to provide a theoretical basis for pilot selection.Methods A total of 14 commissioned pilots(8 fighter pilots,6 transport pilots)and 9 ordinary controls(control group)were randomly selected for arterial spin labeling(ASL),diffusion tensor imaging(DTI)and resting-state functional magnetic resonance imaging(rs-fMRI)to accurately localize differential brain regions between pilots and controls,as well as pilots of different types of aircrafts.Results The ASL results showed that the resting-state CBF value in the left dorsolateral prefrontal lobe of pilots was higher than that of controls(P<0.01),while the resting-state CBF value in the left dorsolateral prefrontal lobe of fighter pilots was higher than that of transport pilots(P<0.01);DTI results showed that the fractional anisotropy(FA)of the corpus callosum and right radial cortex in pilots were higher than those in controls(P<0.01),and the FA of the corpus callosum in fighter pilots were higher than that in transport pilots(P<0.01);the results of rs-fMRI showed that the voxel-mirrored homotopic connectivity(VMHC)values of the occipital lobe,inferior temporal gyrus and paracentral gyrus between the left and right hemispheres of the pilots at resting-state were higher than those of the controls(P<0.01),and the VMHC values of the cerebellum,middle temporal gyrus and insula between the left and right hemispheres of the fighter pilots at resting-state were higher than those of the transport pilots(P<0.05).Conclusion Compared with controls,after long-term flight training,pilots showed increased resting-state CBF in the left dorsolateral prefrontal lobe of the brain,enhanced white matter integrity and brain network connectivity in the resting state.Due to the long-term training of different flight subjects,compared with transport pilots,fighter pilots showed increased resting-state CBF in the left dorsolateral prefrontal lobe of the brain,enhanced white matter integrity and brain network connectivity.
