摘要
目的分析慢性肾脏病(chronic kidney disease,CKD)患者在非高钾血症状态下心电图出现的帐篷状T波的特点。方法选择住院的常规体表心电图诊断为帐篷状T波的高钾血症患者(高钾组)、非高钾血症CKD患者(高钾记忆组)各63例,测量V3导联的T波时限、T波振幅,并比较组间差异。结果两组患者V3导联的T波时限分别为(146.10±20.47)ms、(153.44±16.99)ms,T波振幅分别为1.50(1.00,2.00)mV、1.10(0.70,1.50)mV,组间比较差异均有统计学意义(P均<0.05)。结论帐篷状T波的形态变化与血清钾浓度相关,高钾记忆现象可能与局部心肌钾离子的异常分布或血清钾浓度的动态平衡过程有关。
Objective To analyze the characteristics of tent-shaped T wave in patients with chronic kidney disease(CKD)in a non-hyperkalemia state.Methods We selected 63 hyperkalemia inpatients diagnosed with tent-shaped T wave by routine body surface ECG(hyperkalemia group),and another 63 CKD inpatients without hyperkalemia(hyperkalemia memory group).The duration and amplitude of T wave in lead V3 were measured,and compared between the two groups.Results In the two groups,the T wave duration in lead V3 is separately(146.10±20.47)ms and(153.44±16.99)ms while the amplitude of T wave is 1.50(1.00,2.00)mV and 1.10(0.70,1.50)mV,respectively,both with statistically significant differences(P<0.05).Conclusion The morphological changes of tent-shaped T wave are related to serum potassium concentration.The hyperkalemia memory phenomenon may be associated with the abnormal distribution of potassium ions in local myocardium,or the dynamic balance process of serum potassium concentration.
