摘要
目的研究分析股骨粗隆间骨折(intertrochanteric fracture of femur,IFF)患者应用人工髋关节置换术治疗的效果。方法选择2021年1月—2022年6月在徐州市矿务集团总医院治疗的IFF患者90例,根据患者意愿分成两组,对照组45例,应用股骨近端髓内钉内固定术治疗,研究组45例,应用人工髋关节置换术治疗,对比两组患者手术相关指标、并发症发生情况、髋关节功能、炎性因子水平、生活质量。结果与对照组比较,研究组术中出血量(306.53±22.68)mL更多,下床活动时间(6.61±1.42)d、住院时间(15.26±3.65)d更短,差异有统计学意义(t=17.752、7.668、6.207,P<0.05)。相较于对照组,研究组并发症发生率更低,差异有统计学意义(P<0.05)。与对照组相比,研究组术后15、30 d的髋关节功能评分更高,差异有统计学意义(P<0.05)。相比于对照组,研究组术后各炎症因子水平更低,各生活质量分值更高,差异有统计学意义(P<0.05)。结论IFF患者应用人工髋关节置换术治疗时,患者术后康复速度更快,并发症发生率更低,同时还可以进一步改善患者髋关节功能、生活质量,减轻炎症反应。
Objective To study the effect of artificial hip replacement in patients with intertrochanteric fracture of femur(IFF).Methods A total of 90 IFF patients treated in Xuzhou Mining Group General Hospital from January 2021 to June 2022 were selected and divided into two groups according to the patients'wishes,45 in the control group were treated with proximal femoral intramedullary nail-fixation,and 45 in the study group were treated with artificial hip replacement.Operative indicators,complications,hip function,inflammatory factors and quality of life were compared between the two groups.Results Compared with the control group,the intraoperative blood loss of the study group was more(306.53±22.68)mL,and the time of getting out of bed and staying in hospital were shorter(6.61±1.42)d and(15.26±3.65)d,and the difference was statistically significant(t=17.752,7.668,6.207,P<0.05).Compared with the control group,the complication rate of the study group was lower,and the difference was statistically significant(P<0.05).Compared with the control group,the hip function scores of the study group were higher at 15 d and 30 d after surgery,and the difference was statistically significant(P<0.05).Compared with the control group,the levels of inflammatory factors in the study group were lower after surgery,and the scores of quality of life were higher,and the difference was statistically significant(P<0.05).Conclusion When patients with IFF are treated with artificial hip replacement,the postoperative recovery speed of patients is faster,the complication rate is lower,and the hip function and quality of life of patients can be further improved,
