摘要
葛根芩连汤为《伤寒论》中治疗湿热证的名方,主要用于治疗湿热下利证。随着早中期2型糖尿病(Diabetes Mellitus Type 2,T2DM)湿热型人群比例正逐年升高,将葛根芩连汤应用于该病的治疗也成为当前研究热点。为全面把握葛根芩连汤防治湿热型T2DM的效用及其背后原理,该文对近五年的相关研究进行了梳理分析。临床研究发现,葛根芩连汤联合西药能明显改善患者服药后的不良反应;药理学研究主要集中于葛根素、黄芩素和小檗碱,其中小檗碱有望成为未来防治湿热型T2DM的重点;另外,机制研究表明,葛根芩连汤可通过改善胰岛素抵抗、减轻氧化应激、调节肠道菌群平衡、调节糖脂代谢水平及降低炎症反应等机制调节控制血糖,其中调节肠道菌群为葛根芩连汤日后治疗湿热型T2DM的新靶点。日后可增加葛根芩连汤防治湿热型T2DM的临床研究,并深入挖掘小檗碱的分子机理,进一步研究调控肠道菌群的作用机制。该文系统地阐述了葛根芩连汤防治湿热型T2DM的相关临床研究,及其药理学成分和作用机制,为葛根芩连汤日后临床应用提供科学理论支撑,为中药现代化研究提供参考。
Gegen Qinlian Decoction is a famous prescription in Treatise on Febrile Diseases for the treatment of damp-heat syndrome,mainly used for the treatment of damp-heat syndrome.As the proportion of people with damp heat type 2 diabetes(Diabetes Mellitus Type 2,T2DM)in the early and middle stages is increasing year by year,the application of Gegen Qinlian Decoction in the treatment of this disease has become a current research hotspot.In order to comprehensively grasp the effectiveness of Gegen Qinlian Decoction in preventing and treating damp-heat T2DM and the underlying principles,this article sorts out and analyzes the relevant research in the past five years.Clinical studies have found that Gegen Qinlian Decoction combined with western medicine can significantly improve the adverse reactions of patients after taking the medicine;pharmacological studies mainly focus on puerarin,baicalein and berberine,of which berberine is expected to become the focus of future prevention and treatment of damp-heat T2DM;In addition,mechanism studies have shown that Gegen Qinlian Decoction can regulate and control blood sugar through mechanisms such as improving insulin resistance,alleviating oxidative stress,regulating the balance of intestinal flora,regulating the level of glucose and lipid metabolism,and reducing inflammation.Among them,the regulation of intestinal flora is Gegen Qinlian Decoction is a new target for the future treatment of damp-heat type T2DM.In the future,we can increase the clinical research on the prevention and treatment of damp-heat T2DM by Gegen Qinlian Decoction,and dig deeper into the molecular mechanism of berberine to further study the mechanism of regulating intestinal flora.This article systematically expounds the relevant clinical research of Gegen Qinlian Decoction in preventing and treating damp-heat type T2DM,as well as its pharmacological components and mechanism of action,in order to provide scientific theoretical support for the future clinical application of Gegen Qinlian Decoction and provide reference for the modernization of traditional Chinese medicine.
