摘要
目的:系统评价音乐疗法在结肠镜检查过程中对受检者的影响。方法:计算机检索PubMed、EMbase、Web of Science、PsycINFO、Cochrane Library、CINAHL、万方数据库(WanFang Data)、中国生物医学文献数据库(CBM)、中国知网(CNKI)和维普(VIP)数据库中关于音乐在结肠镜检查患者中干预效果的随机对照试验,检索时限为建库至2022年10月31日。两名研究者独立按照纳入和排除标准筛选文献和提取资料,RevMan 5.4软件进行Meta分析,Cochrane协作网的偏倚风险评价工具进行纳入文献质量评价和偏倚风险评估。结果:共纳入17项随机对照研究,共2969例患者。Meta分析结果显示,音乐疗法能缓解患者在结肠镜检查过程中的疼痛(MD=-1.21,95%CI-2.04~-0.38,P<0.01)、焦虑(MD=-3.12,95%CI-3.34~-2.90,P<0.01),缩短结肠镜检查的进镜时间(MD=-2.62,95%CI-3.43~-1.81,P<0.01)和持续时间(MD=-2.93,95%CI-5.33~-0.54,P<0.05),缓解患者在检查过程中心率的波动(MD=-7.91,95%CI-11.25~-4.57,P<0.01),提升患者满意度(MD=-0.61,95%CI-0.82~-0.41,P<0.01),但对检查中患者的收缩压(MD=-0.31,95%CI-0.85~-0.23,P>0.05)与舒张压(MD=-0.03,95%CI-0.30~0.23,P>0.05)的影响,差异无统计学意义。结论:音乐疗法能够有效缓解患者在结肠镜检查过程中的疼痛和焦虑,明显缩短结肠镜检查的进镜时间和检查持续时间,降低了患者在检查过程中心率的波动,提升了患者满意度,建议临床推广应用。
Objective:Systematic evaluation of the impact of music therapy on subjects during colonoscopy.Methods:Search PubMed,EMbase,Web of Science,PsycINFO,Cochrane Library,CINAHL,WanFang Data,CBM,CNKI,and VIP databases for randomized controlled trials on the effectiveness of music intervention in patients undergoing colonoscopy.The search was conducted until October 31,2022.Two researchers independently selected literature and extracted data based on inclusion and exclusion criteria.RevMan 5.4 software was used for meta-analysis,and the bias risk assessment tool of the Cochrane collaborative network was used for inclusion literature quality evaluation and bias risk assessment.Results:A total of 17 randomized controlled studies were included,with a total of 2969 patients.Meta analysis results showed music therapy can alleviate patients'pain(MD=-1.21,95%CI-2.04--0.38,P<0.01),anxiety(MD=-3.12,95%CI-3.34--2.90,P<0.01),shorten the entry time(MD=-2.62,95%CI-3.43--1.81,P<0.01)and duration(MD=-2.93,95%CI-5.33--0.54,P<0.05)during colonoscopy,alleviate the fluctuation of the patient's heart rate during the examination process(MD=-7.91,95%CI-11.25--4.57,P<0.01),improve patient satisfaction(MD=-0.61,95%CI-0.82--0.41,P<0.01),but have no statistically significant impact on the systolic blood pressure(MD=-0.31,95%CI-0.85--0.23,P>0.05)and diastolic blood pressure(MD=-0.03,95%CI-0.30-0.23,P>0.05)of the patients during the examination.Conclusion:Music therapy can effectively alleviate patients'pain and anxiety during colonoscopy,significantly shorten the entry time and duration of colonoscopy,reduce the fluctuation of patients'center rate during the examination process,and improve patient satisfaction.It is recommended to promote clinical application.
