新生儿重症监护室中极低和超低出生体重儿院内感染影响因素分析 被引量:7

Analysis on the Influencing Factors of Nosocomial Infection in Very Low and Ultra-low Birth Weight Infants in Neonatal Intensive Care Unit

摘要 目的通过分析某院新生儿重症监护室中极低出生体重儿和超低出生体重儿院内感染的影响因素,为临床预防提供数据依据。方法回顾性分析某院2020年1月1日-2023年1月31日收治的118例极低出生体重儿和21例超低出生体重儿临床资料。单因素分析极低、超低出生体重儿两组间院内感染与胎龄、出生体重、住院时长、住院费用、并发症及有创操作之间的关系。对组间有差异的因素进行多因素Logistic回归分析。结果收治的极低、超低体重儿共139例,其中极低出生体重儿118例,发生院内感染103例,超低出生体重儿21例,发生院内感染20例。院内感染疾病依次为新生儿肺炎、新生儿败血症、泌尿系感染、结膜炎、新生儿化脓性脑膜炎、肠炎。经Logistic回归分析显示胎龄≤30周、体重<1000克、经外周静脉穿刺中心静脉置管留置时长>10天、机械通气是发生院内感染的危险因素,差异具有统计学意义,P<0.05;院内感染增加会延长住院时间,增加住院费用,组间比较差异具有统计学意义。结论极低和超低出生体重儿的院内感染的发生率高,严栺消毒、无菌操作、减少侵袭性操作是预防新生儿重症监护室院内感染的重要途径。 Objectives To analyze the influencing factors of nosocomial infection of very low birth weight infants and ultra-low birth weight infants in neonatal intensive care unit of a hospital,and provide data basis for clinical prevention.Methods Clinical data of 118 very low birth weight infants and 21 ultra-low birth weight infants admitted to a hospital from January 1st,2020 to January 31st,2023 were retrospectively analyzed.Univariate analysis was conducted to determine the relationship between nosocomial infection and gestational age,birth weight,length of stay,hospitalization cost,complications and invasive procedures in very low and ultra-low birth weight infants.Multivariate Logistic regression analysis was performed for the factors with differences between the groups.Results A total of 139 cases of very low birth weight and ultra-low birth weight infants were admitted,including 118 cases of very low birth weight infants,103 cases of nosocomial infection,21 cases of ultra-low birth weight infants,20 cases of nosocomial infection.Nosocomial infections were neonatal pneumonia,neonatal sepsis,urinary tract infection,conjunctivitis,neonatal suppurative meningitis,enteritis.Logistic regression analysis showed that gestational age≤30 weeks,body weight<1000 g,central venous catheter retention time>10 days through peripheral venipentesis and mechanical ventilation were risk factors for nosocomial infection,with statistical significance(P<0.05).The increase of nosocomial infection would prolong hospital stay and increase hospitalization cost,and the difference between groups was statistically significant.Conclusions The incidence of nosocomial infection was high in very low and ultra-low birth weight infants.Strict disinfection,aseptic operation and reducing invasive operation were important ways to prevent nosocomial infection in neonatal intensive care unit.
