摘要
目的分析某肿瘤专科医院主要诊断类别为消化系统疾病及功能障碍,代码MDCG(Diseases and Disorders of the Digestive System)组中手术病例院内感染疾病的负担情况。方法回顾性分析2017年研究医院MDCG组院内感染病例与非院内感染病例的时间消耗指数及费用消耗指数,分析院内感染病例的疾病负担。结果2017年MDCG组手术病例中,院内感染病例组的时间消耗指数和费用消耗指数分别是非院内感染病例组的1.98倍和1.42倍,院内感染显著增加了患者的负担。院内感染病例主要集中在GB11、GB15和GB23三个组,三个DRG组中院内感染病例组的平均住院日和次均费用均高于非院内感染病例组(p<0.05)。结论院内感染显著增加了手术患者的住院费用,降低了医院的服务效率,应进一步加强手术患者院内感染防控,加强对院内感染发生率高的病种和科室的监控。
Objective To explore the nosocomial infection burden of the surgical cases in the Diseases and Disorders of the Digestive System group(coded as MDCG)of a cancer hospital.Methods Retrospective analysis of the time consumption index and cost consumption index of nosocomial infection and non-nosocomial infection cases in the MDCG group of the hospital in 2017 was conducted,to analyze the nosocomial infection burden.Results Among the surgical cases in the MDCG group in 2017,the time consumption index and cost consumption index of the nosocomial infection group were 1.98 times and 1.42 times that of the non-nosocomial infection group,respectively.The infection significantly increased the burden on patients.Nosocomial infection cases mainly happened in the three groups of GB11,GB15 and GB23.In the three groups,the average length of stay and cost per hospitalization of the nosocomial infection cases were higher than those of the non-nosocomial infection cases(p<0.05).Conclusion Nosocomial infections significantly increase hospitalization costs for surgical patients and reduce hospital service efficiency.It is necessary to further strengthen the prevention and control of nosocomial infections in surgical patients and to strengthen the monitoring of diseases and departments with higher incidence.
