摘要
目的:研究舌咽三步针联合治疗脑卒中假性球麻痹吞咽功能障碍的效果.方法:选取本院收治的98例脑卒中假性球麻痹吞咽功能障碍的患者为研究对象.采用随机数字表法将其分为研究组(n=49)和常规组(n=49).研究组在给予舌压抗阻反馈训练治疗的基础上给予舌咽三步针治疗,常规组仅给予舌压抗阻反馈训练治疗.比较两组患者的临床疗效、治疗前、后洼田饮水试验、标准吞咽功能评价量表(SSA)评分、表面肌电图指标等.结果:研究组临床总有效率93.88%(46/49)高于常规组79.59%(39/49),差异具有统计学意义(P<0.05).治疗后与常规组比较,研究组洼田饮水试验评分较高,SSA评分较低,舌骨上肌群、舌骨下肌群吞咽时程较短,舌骨上肌群、舌骨下肌群最大波幅值较高,差异均有统计学意义(P<0.05).结论:舌咽三步针联合舌压抗阻反馈训练治疗脑卒中假性球麻痹吞咽功能障碍患者的效果确切,可有效提升吞咽功能,减轻吞咽障碍.
Objective:To study the effect of three-step glossopharyngeal acupunctures in the treatment of patients with swallowing dysfunction in stroke pseudobulbar palsy.Methods:Ninety-eight patients admitted to our hospital with stroke pseudobulbar palsy with swallowing dysfunction were selected and divided into a study group(n=49)and a conventional group(n=49)using the random number table method.The study group was given three-step glossopharyngeal acupunctures treatment combined with tongue pressure resistance feedback training.The conventional group was given tongue pressure resistance feedback training treatment only.The clinical efficacy,pudding field drinking test,standard swallowing assessment scale(SSA)score,and surface electromyography were compared between the two groups before and after treatment.Results:The total clinical efficiency of the study group was 93.88%(46/49),which was higher than that of the conventional group[79.59%(39/49)],and the difference was statistically significant(P<0.05).After treatment,the study group had a higher puddle drinking test score and a lower SSA score than the conventional group(P<0.05).And after treatment,compared with the conventional group,the study group had a shorter swallowing time for the supraglottis and subglottis muscle groups and higher maximum wave amplitude values for the supraglottis and subglottis muscle groups(P<0.05).Conclusion:Three-step glossopharyngeal acupunctures combined with tongue pressure resistance feedback training is effective in treating swallowing dysfunction in stroke pseudobulbar palsy,which can enhance swallowing function and reduce swallowing disorders.
