摘要
目的:探析右美托咪定对糖尿病患者腹腔镜肠道手术中血糖及肾功能的影响。方法:将2021年11月—2022年11月古田县医院收治的100例糖尿病腹腔镜肠道手术患者随机分成A组与B组,各50例。A组采用全凭静脉麻醉,B组采用右美托咪定联合全凭静脉麻醉,观察两组的血流动力学(平均动脉压、心率)、血糖水平、肾功能(尿素氮、肌酐)、认知功能[简易智力状态检查表(MMSE)评分]及不良反应(口干、心动过缓、恶心呕吐、急性肾损伤)。结果:B组气管插管时、切片时、术毕时、拔除气管时的平均动脉压、心率均低于A组(P<0.05);B组气管插管时、切片时、术毕时、拔除气管时的血糖水平均低于A组(P<0.05);B组术后1 d的尿素氮、肌酐均高于A组(P<0.05);B组术后1 d的MMSE评分高于A组(P<0.05);B组不良反应发生率为12.00%,与A组的16.00%相近(P>0.05)。结论:右美托咪定对糖尿病患者腹腔镜肠道手术中的血流动力学、血糖水平及肾功能有一定的保护作用,且可减轻麻醉对认知功能的影响,不增加不良反应,有效性与安全性均较好。
Objective:To explore the effect of Dexmedetomidine on blood glucose and renal function in patients with diabetes undergoing laparoscopic intestinal surgery.Method:A total of 100 patients with diabetes undergoing laparoscopic intestinal surgery in Gutian County Hospital from November 2021 to November 2022 were randomly divided into the group A and the group B.The group A was treated with total intravenous anesthesia,the group B was treated with Dexmedetomidine combined with total intravenous anesthesia.Hemodynamics(mean arterial pressure,heart rate),blood glucose level,renal function(urea nitrogen,creatinine),cognitive function(MMSE score)and adverse reactions(hypotension,bradycardia,nausea and vomiting,acute renal injury)of the two groups were observed.Result:The mean arterial pressure and heart rate in the group B were lower than those in the group A at the time of endotracheal intubation,section,operation and extubation(P<0.05);the blood glucose level in the group B was lower than that in the group A at the time of endotracheal intubation,section,operation and extubation(P<0.05);urea nitrogen and creatinine in the group B were higher than those in the group A on the first day after operation(P<0.05);the MMSE score of the group B was higher than that of the group A on the first day after operation(P<0.05);the incidence of adverse reactions in the group B was 12.00%,which was similar to 16.00%in the group A(P>0.05).Conclusion:Dextrmedetomidine has a protective effect on hemodynamics,blood glucose level and renal function during laparoscopic intestinal surgery in patients with diabetes,and can reduce the impact of anesthesia on cognitive function without increasing adverse reactions.It is effective and safe.
