摘要
目的探讨富血小板纤维蛋白(PRF)联合薄中厚皮片植皮治疗小面积深度皮肤损伤的效果。方法本研究为病例对照研究,选取2016年5月至2020年5月北部战区总医院烧伤整形科收治的41例由艾灸导致的三度烧伤患者,男20例,女21例,年龄(54.7±16.2)岁,年龄范围为38~70岁。采用随机数表法将患者随机分为常规组(n=20)和PRF组(n=21)。两组患者在局部麻醉下行手术切痂,去除坏死的皮肤软组织,保留皮下组织。常规组患者术后创面外敷凡士林油纱包扎。PRF组患者创面外敷PRF凡士林油纱包扎。比较两组患者的植皮成活情况、术后不良反应、植皮区颜色、换药次数、治疗时间和治疗费用。结果PRF组植皮成活率为100%(21/21),移植皮片接近正常皮肤颜色,无瘢痕增生。常规组植皮皮片成活率为95%(19/20),植皮皮片呈粉红色,瘢痕增生。常规组患者的治疗时间[(28.05±10.13)d]高于PRF组[(20.27±4.33)d],差异有统计学意义(P<0.05)。常规组住院费用[(8164.26±1998.83)元]高于PRF组[(6494.54±1976.42)元],差异有统计学意义(P<0.05)。结论PRF联合薄中厚皮片治疗局部艾灸烧伤创面的治疗时间短,移植皮片成活率高,术后无水疱及瘢痕增生发生。
Objective To investigate the clinical effect of platelet rich fibrin(PRF)combined with thin and medium thick skin grafting in the treatment of deep skin lesion.Methods This study was a case-control study,a total of 41 patients with third-degree burns caused by moxibustion,20 males and 21 females,aged(54.7±16.2)years old,ranging from 38 to 70 years old,were selected from May 2016 to May 2020 in the department of Burn and Plastic Surgery of General Hospital of Northern Theater Commmand.Patients were divided into conventional group(n=20)and PRF group(n=21)by random number table.In both groups,the scab was excised under local anesthesia to remove the necrotic skin and soft tissue and preserve the subcutaneous tissue.The wound of conventional group was bandaged with petroleum jelly.The wound of observation group was bandaged with PRF Vaseline gauze.The survival rate,adverse reaction,color of skin graft area,frequency of dressing change,treatment time and cost were compared between the two groups.Results The survival rate of skin graft in PRF group was 100%(21/21),and the grafts were close to normal skin color without scar hyperplasia.The survival rate of the graft in the conventional group was 95%(19/20).The graft was pink with scar hyperplasia.The treatment time of conventional group[(28.05±10.13)d]was higher than that of PRF group[(20.27±4.33)d],and the difference was statistically significant(P<0.05).The hospitalization cost in the conventional group[(8164.26±1998.83)yuan]was higher than that in the PRF group[(6494.54±1976.42)yuan],and the difference was statistically significant(P<0.05).Conclusions In the treatment of local moxibustion burn wounds,PRF combined with thin,medium and thick skin slices has a short treatment time,a high survival rate of skin grafts,no postoperative blisters and scar hyperplasia.
