摘要
目的分析支持性心理干预在烧伤整形植皮中的应用效果,评价其对患者术后疼痛感与负性情绪的影响。方法选择2021年1月~2022年2月于哈尔滨医科大学附属第一医院整形美容中心进行烧伤整形植皮的60例患者,采用随机数字表法分为对照组和研究组,每组30例。对照组实施常规干预,研究组实施支持性心理干预,比较各组手术效果、疼痛情况、情绪状态、社会支持和应对方式。结果研究组术后皮下血肿个数显著少于对照组,瘢痕指数评分低于对照组,创面愈合时间短于对照组,植皮成活创面覆盖率高于对照组(P<0.05);研究组各项症状自评量表(Symptom Checklist-90,SCL-90)评分均显著低于对照组(P<0.05);研究组干预后3 d、7 d、30 d的疼痛评分均显著低于对照组(P<0.05);研究组各项社会支持评定量表(Social Support Rate Scale,SSRS)评分均显著高于对照组(P<0.05);研究组应对积极因子评分均显著高于对照组,消极因子评分均显著低于对照组(P<0.05)。结论对烧伤整形植皮患者实施支持性心理干预可明显改善患者负性情绪、减轻疼痛并促进创面愈合,同时还可提高患者社会支持度,可在临床应用推广。
Objective To analyze the application effect of supportive psychological intervention in patients with burn plastic skin grafting,and to evaluate its effect on postoperative pain and negative emotions.Methods Sixty patients who underwent burn plastic skin grafting at the Plastic and Cosmetic Center of the First Affiliated Hospital of Harbin Medical University from January 2021 to February 2022 were selected.They were divided into control group and research group by random number table method,with 30 cases in each group.Routine intervention was implemented in the control group,and supportive psychological intervention was implemented in the research group,and the surgical effects,pain conditions,emotional state,social support and coping styles were compared.Results The number of postoperative subcutaneous hematomas in the research group was significantly less than that in the control group,the scar index score was lower than that in the control group,the wound healing time was shorter than that in the control group,and the coverage rate of skin graft survival was higher than that in the control group(P<0.05);The scores of Symptom Checklist-90(SCL-90)were significantly lower than those of the control group(P<0.05);The pain scores of 3 d,7 d,and 30 d after the intervention of the research group were significantly lower than those of the control group(P<0.05);The Social Support Rate Scale(SSRS)scores of the research group were significantly higher than those of the control group(P<0.05);The scores of positive factors in the research group were significantly higher than those of the control group,and the scores of negative factors were significantly lower than the control group(P<0.05).Conclusion Supportive psychological intervention for patients with burn plastic skin grafting can significantly improve patients’negative emotions,reduce pain and promote wound healing.It can also improve the social support of patients,which can be applied and promoted in clinical practice.
