摘要
目的探讨超脉冲CO_(2)激光联合磁敷疗法治疗浅表型增殖期婴幼儿血管瘤(IH)的临床效果及其对患儿血清表皮生长因子样结构域蛋白7(EGFL7)、缺氧诱导因子1α(HIF-1α)、基质金属蛋白酶9(MMP9)、组织金属蛋白酶抑制剂1(TIMP-1)水平的影响。方法将110例浅表型增殖期IH患儿随机分成观察组与对照组,各55例。观察组采用超脉冲CO_(2)激光联合磁敷疗法治疗,对照组采用单纯超脉冲CO_(2)激光治疗。比较两组患儿近期与远期治疗有效率,以及治疗前后的血清EGFL7、HIF-1α、MMP9、TIMP-1水平,观察两组患儿治疗期间不良反应的发生情况。结果观察组近期治疗有效率与远期治疗有效率均高于对照组(均P<0.05)。治疗后,两组患儿的血清EGFL7、HIF-1α、MMP9水平均低于治疗前,且观察组患儿的上述指标水平均低于对照组(均P<0.05);两组患儿的血清TIMP-1水平均高于治疗前,且观察组患儿的血清TIMP-1水平高于对照组(均P<0.05)。两组患儿治疗期间局部肿胀、局部红斑、色素沉着的发生率比较,差异均无统计学意义(均P>0.05)。结论相比于单纯超脉冲CO_(2)激光治疗,超脉冲CO_(2)激光联合磁敷疗法可提高浅表型增殖期IH患儿的治疗效果,安全性较高,其机制可能与调节血清EGFL7、HIF-1α、MMP9和TIMP-1水平有关。
Objective To investigate the clinical effect of ultrapulsed CO_(2) laser combined with magnetic application therapy for the treatment of superficial and proliferative infantile hemangioma(IH),and its influence on children′s serum epidermal growth factor-like domain protein 7(EGFL7),hypoxia inducible factor 1α(HIF-1α),matrix metalloproteinase 9(MMP9),tissue inhibitor of metalloproteinase 1(TIMP-1)levels.Methods A total of 110 children with superficial and proliferative IH were randomly divided into observation group or control group,with 55 cases in each group.The observation group was treated with ultrapulsed CO_(2) laser combined with magnetic application therapy,whereas the control group was treated with ultrapulsed CO_(2) laser alone.The short-and long-term effective rates of treatment,and pre-and post-treatment serum EGFL7,HIF-1α,MMP9,and TIMP-1 levels of children were compared between the two groups.The occurrence of adverse reactions during treatment was observed in children of both groups.Results The short-and long-term effective rates of treatment in the observation group were higher than those in the control group(all P<0.05).After treatment,the levels of serum EGFL7,HIF-1α,and MMP9 of the two groups were lower than those before treatment,and the observation group yielded lower levels of aforesaid indices as compared with the control group(all P<0.05);furthermore,the level of serum TIMP-1 of the two groups was higher than that before treatment,and the observation group interpreted a higher serum TIMP-1 level as compared with the control group(all P<0.05).During treatment,there was no statistically significant difference in the incidence rates of localized swelling,localized erythema,and hyperpigmentation of children between the two groups(all P>0.05).Conclusion Compared with single ultrapulsed CO_(2) laser therapy,ultrapulsed CO_(2) laser combined with magnetic application therapy can improve therapeutic effect of children with superficial and proliferative IH,exerting a relatively high safety,and its mechanism may be related to the regulation of serum EGFL7,HIF-1α,MMP9,and TIMP-1 levels.
