机器人辅助步态训练对脑卒中患者步态影响的Meta分析 被引量:4

Effect of robot-assisted gait training on the gait of stroke patients:a Meta-analysis of randomised controlled trials

摘要 目的评价机器人辅助步态训练对脑卒中患者步态功能的干预效果,为其制定下肢功能锻炼方案提供循证依据。方法计算机检索Cochrane Library、PubMed、Web of Science、Embase、中国生物医学文献数据库(CBM)、中国知网、维普、万方8个中英文数据库,纳入从建库至2022年5月符合纳入标准与排除标准的随机对照试验,进行质量评价及资料提取后采用RevMan5.3软件进行Meta合并数据分析。结果共纳入22项随机对照试验,总计902例患者,文献质量等级均为B级。Meta分析结果显示,干预组机器人辅助步态训练对脑卒中患者的步速[MD=0.08,95%CI(0.06,0.09),P<0.001]、步长[MD=3.58,95%CI(2.01,5.15),P<0.001]、步频[SMD=0.77,95%CI(0.36,1.18),P<0.001]、步幅[MD=2.35,95%CI(1.12,3.57),P<0.001]、耐力[MD=26.75,95%CI(6.89,46.60),P=0.008]、平衡功能[MD=3.29,95%CI(1.64,4.94),P<0.001]、下肢运动功能[MD=3.04,95%CI(1.73,4.35),P<0.001]指标其干预效果均优于对照组,但对步宽的干预两组比较,差异无统计学意义(P>0.05)。结论机器人辅助步态训练可在一定程度上提高脑卒中患者的步速、步频和耐力,增加步长及步幅,改善平衡及下肢运动功能;而对步宽的改善效果尚不明显。 Objective To evaluate the intervention effect of robot-assisted gait training on the gait of stroke patients so as to provide evidence-based evidence for formulation of lower limb function exercise programs.Methods Eight Chinese and English databases including Cochrane Library,PubMed,Web of Science,Embase,China Biomedical Literature Database(CBM),China National Knowledge Network,VIP and Wanfang Database were searched from the inceptions to May 2022 to retrieve the randomised controlled trials meeting the inclusion criteria.After quality evaluation and data extraction,Meta-analysis of the combined data was performed using RevMan5.3 software.Results A total of 22 randomised controlled trials with 902 patients were included.The retrieved data were rated B.Meta-analysis showed that robot-assisted gait training in the intervention group had significant effects on stroke patients,in step speed [MD=0.08,95%CI(0.06,0.09),P<0.001],step length[MD=3.58,95%CI(2.01,5.15),P<0.001],step frequency[SMD=0.77,95%CI(0.36,1.18),P<0.001],stride length[MD=2.35,95%CI(1.12,3.57),P<0.001],endurance[MD=26.75,95%CI(6.89,46.60),P=0.008],balance function[MD=3.29,95%CI(1.64,4.94),P<0.001]and lower limb motor function[MD=3.04,95%CI(1.73,4.35),P<0.001].The effect of intervention was better than that of the control group,except the step width(P>0.05).Conclusion The robot-assisted training can improve balance and lower limb motor function by ways of improving stroke patients'step speed,stride frequency and endurance and increase step length and stride length,but it fails to significantly improve step width.
