磁共振功能成像联合尿8-羟基2脱氧鸟苷、Ⅳ型胶原检测对党参改善糖尿病肾病肾功能用量的研究

Study on the Dosage of Codonopsis Pilosula in Improving Renal Function by Functional MRI Combined with Urinary 8-OHdG and IV Collagen Detection

摘要 目的:探讨磁共振功能成像联合尿8-羟基2脱氧鸟苷(8-OHdG)、Ⅳ型胶原指标评估党参对糖尿病肾病(DN)患者肾功能的影响。方法:选取64例DN患者,党参用药低剂量组、中等剂量组、高剂量组及对照组(20例),各组用药后4周、8周、12周末,使用核磁共振功能成像对各组受试者进行肾脏MRI扫描及尿液分析。比较用药后肾脏MRI表现、尿8-OHdG、Ⅳ型胶原水平。结果:高剂量组糖尿病肾病ADC值较用药前明显升高,差异显著具有统计学意义(P<0.05);低剂量组及中等剂量组ADC值变化不具有统计学意义。低剂量组及中等剂量组尿8-OHdG、Ⅳ型胶原水平均高于高剂量组,差异显著具有统计学意义(P<0.05)。低剂量尿8-OHdG、Ⅳ型胶原水平与中等剂量组对照差异不明显,不具有统计学意义。结论:MRI联合尿8-OHdG、Ⅳ型胶原水平检测可以有效评估不同剂量党参用药对糖尿病肾病患者肾功能的影响;高剂量党参用药可以改善糖尿病肾病患者肾功能。 Objective:to evaluate the effect of codonopsis Pilosula on renal function in patients with diabetic nephropathy by functional magnetic resonance imaging combined with urinary 8-OHdG andⅣcollagen markers.Methods:64 patients with DN were selected,including low dose Group,middle-dose Group,high-dose Group and Control Group(20),MRI Scans of kidneys and urine analysis were performed using functional magnetic resonance imaging.MRI manifestations of kidney,urinary 8-OHdG andⅣCollagen levels were compared.Results:the ADC values of high dose group were significantly higher than those of pre-treatment group(P<0.05),and the ADC values of low dose group and Middle Dose Group were not significantly higher than those of pre-treatment group(P<0.05)。The levels of urinary 8-0HdG,typeⅣCollagen in low-dose Group and middle-dose group were significantly higher than those in high-dose group(P<0.05).There was no significant difference in 8-0HdG,Ⅳcollagen level between low-dose Group and middle-dose Group.Conclusion:MRI combined with urine 8-OHdG andⅣCollagen levels can flfctively evaluate the effects of different doses of codonopsis pilosula on renal function in patients with diabetic nephropathy,and high doses of Codonopsis Pilosula can improve renal function in patients with diabetie nephropathy.
