摘要
目的探讨PDCA循环管理法在改善医院专科DRG指标中的运用成效。方法选取某三甲医院6个不同临床专科2020年1月1日-2021年12月31日的住院病案首页信息,采用国家版CN-DRG分组器分组,共入组9782例。运用PDCA循环管理法对2021年1月1日-2021年12月31日DRG指标数据进行持续改进,对比分析未实施PDCA循环管理的2020年1月1日-2020年12月31日DRG指标数据。结果运用PDCA循环管理后,6个专科的入组率均大于99%,平均增长率为2.04%;DRG组数均增加,共增加了55个DRG组,平均增长率为18.33%;总权重均增长,平均增长率为39.60%;5个专科的CMI值有增长,1个专科的CMI值与实施前持平,平均增长率为5.14%;时间消耗指数均有所降低,平均下降6.93%;费用消耗指数平均下降率为5.93%。结论运用PDCA循环管理方法,持续改善专科DRG指标,有利于加强专科医疗质量管理、提高医疗技术水平、缩短住院时间、控制住院医疗费用,提升医疗服务能力。
Objectives This study aims to explore the application effect of PDCA circular management method in improving the DRG index of clinical specialties in hospitals.Methods The information on the front page of inpatient medical records of six different clinical specialties in a tertiary hospital from January 1,2020 to December 31,2021 was selected and grouped with the national version of the CN-DRG grouping device.A total of 9782 cases were included in the group.The PDCA cycle management method was used to continuously improve the DRG index data from January 1,2021 to December 31,2021,and the DRG index data from January 1,2020 to December 31,2020 without PDCA cycle management were compared and analyzed.Results After PDCA circulation management,the enrollment rate of the six clinical specialties was more than 99%,and the average growth rate was 2.04%.The number of DRG groups increased,and a total of 55 DRG groups were added,with an average growth rate of 18.33%.The total weight increased,with an average growth rate of 39.60%.The CMI value of 5 specialties increased,and the CMI value of 1 specialty remained the same as before the implementation,with an average growth rate of 5.14%.The time consumption index decreased,with an average decrease of 6.93%.The average decline rate of the cost consumption index was 5.8%.Conclusions Using PDCA cycle management method to continuously improve the DRG index of clinical specialties is conducive to strengthening medical quality management,improving medical technology level,shortening hospitalization time,controlling hospitalization medical expenses,and improving medical service ability.
