摘要
目的探讨血清淀粉样蛋白A(SAA)与肿瘤标志物联合检测在肺癌诊断中的应用效果。方法将2019年6月—2021年6月于菏泽医学专科学校附属医院诊治的70例肺癌患者纳入肺癌组,将同期收治的73例良性肺疾病患者纳入良性对照组,同期将在本院体检的80名健康体检者纳入健康对照组。抽取全部研究对象的5 mL静脉血,检测并比较3组的SAA、肿瘤标志物癌胚抗原(CEA)、糖类抗原125(CA125)、神经元特异性烯醇化酶(NSE)水平;同时描绘受试者工作曲线(ROC),分析SAA、CEA、CA125、NSE单独与联合检测在肺癌中的诊断价值。结果肺癌组的SAA(38.69±3.87)mg/L、CEA(5.23±0.71)μg/L、CA125(39.67±6.33)KU/L、NSE(17.22±2.75)μg/L水平高于良性对照组与健康对照组,差异有统计学意义(F=2953.926、828.572、417.486、431.717,P<0.05);ROC结果显示:SAA、CEA、CA125、NSE单独检测在肺癌诊断中的曲线下面积(AUC)分别为0.859、0.885、0.783、0.779,而四项指标联合检测的AUC为0.927,高于四项指标单独检测。结论SAA联合肿瘤标志物能够提高肺癌检测的精准度,对肺癌的早期鉴别诊断具有重要意义。
Objective To explore the effect of combined detection of serum amyloid A(SAA)and tumor marker in the diagnosis of lung cancer.Methods 70 patients with lung cancer who were treated in the affiliated hospital of Heze Medical College from June 2019 to June 2021 were included in the lung cancer group,73 patients with benign lung disease who were treated in the same period were included in the benign control group,and 80 healthy people who were examined in our hospital were included in the healthy control group.Take 5 mL of venous blood from all the subjects,detect and compare the levels of SAA,tumor markers carcinoembryonic antigen(CEA),carbohydrate antigen 125(CA125),neuron specific enolase(NSE)in the three groups;at the same time,the subject working curve(ROC)was drawn to analyze the diagnostic value of SAA,CEA,CA125,NSE alone and in combination in lung cancer.Results The levels of SAA(38.69±3.87)mg/L,CEA(5.23±0.71)μg/L,CA125(39.67±6.33)KU/L,NSE(17.22±2.75)μg/L in lung cancer group were higher than those in benign control group and healthy control group,and the difference was statistically significant(F=2953.926,828.572,417.486,431.717,P<0.05).The ROC results showed that the area under the curve(AUC)of SAA,CEA,CA125 and NSE in the diagnosis of lung cancer were 0.859,0.885,0.783,0.779,respectively,while the AUC of the combined detection of the four indicators was 0.976,which was higher than that of the independent detection of the four indicators.Conclusion SAA combined with tumor markers can improve the accuracy of lung cancer detection,which is important for early differential diagnosis of lung cancer.
