摘要
目的 探讨多普勒超声辅助在早期乳腺癌患者中的诊断价值及与病理诊断的一致性。方法 选取2019年8月-2022年8月在东莞市东部中心医院检查的65例疑似乳腺癌的女性作为观察对象,对比其最终病理学检测结果,均对患者进行多普勒超声检查,分析超声检测在病症检测中的诊断效应,包括敏感度、特异度和准确度,以此分析多普勒超声和病理诊断的一致性。结果 65例疑似乳腺癌患者中,病理检出恶性60例,良性5例,多普勒超声检出疑似恶性58例,良性7例,敏感度、特异度和准确度分别为93.33%、60.00%、90.77%;分析超声检查征象,不规则形态检出57例,椭圆形形态检出8例;肿块边缘不光滑平整检出56例,光滑平整检出9例;有低回声团52例;内见强光点或伴有钙化13例;多普勒超声征象与病理诊断在肿瘤良恶性鉴别中具有较高的一致性(Kappa>0.600)。结论 多普勒超声用于早期乳腺癌筛查中具有较高的检出灵敏度、特异度及准确度。与病理诊断结果具有一定的一致性,但是乳腺钙化、病灶直径、后方及内部回声是引起乳腺癌误诊或漏诊的重要因素。
Objective To investigate the diagnostic value of Doppler-assisted ultrasound in patients with early breast cancer and its consistency with pathological diagnosis.Methods 65 cases of women with suspected breast cancer who were examined in Dongguan Eastern Central Hospital from August 2019 to August 2022 were selected as the observation objects,and the final pathological test results were compared.The diagnostic effect of ultrasonography in disease detection,including sensitivity,specificity,and accuracy were compared to analyze the consistency of Dolap ultrasonography and pathological diagnosis.Results Among the 65 patients with suspected breast cancer,60 were malignant and 5 were benign,and 58 suspected malignant and 7 benign were detected by Dolap ultrasonography.The sensitivity,specificity and accuracy were 93.33%,60.00%and 90.77%respectively.Ultrasound examination signs were analyzed,57 cases were detected with irregular shape and 8 cases were detected with oval shape;56 cases were detected with uneven and smooth edges of the mass,and 9 cases were detected with smooth and flat;52 cases had hypoechoic mass;in 13 cases,strong light spots were seen or accompanied by calcification;Doppler ultrasound signs and pathological diagnosis had high consistency in the differentiation of benign and malignant tumors(Kappa>0.600).Conclusion Doppler ultrasound has high detection sensitivity,specificity and accuracy in early breast cancer screening.It is consistent with the pathological diagnosis results,but breast calcification,lesion diameter,posterior and internal echo are important factors that cause breast cancer misdiagnosis or missed diagnosis.
