摘要
目的探讨食管癌术后患者病耻感、心理弹性、自我感受负担与生活质量的相关性。方法选取2020年1月—2021年5月在安阳市肿瘤医院首次接受食管癌手术治疗的100例患者为研究对象,采用社会影响量表(SIS)、心理弹性量表(CD-RISC)、自我感受负担量表(SPBS)及生活质量测定量表(EORCT QLQ-C30)对患者进行调查,采用Pearson相关分析患者术后病耻感、心理弹性、自我感受负担与生活质量的相关性。结果食管癌患者术后SIS中经济歧视、社会隔离、内在羞耻感、社会排斥4个维度评分及总分均高于国内常模,差异均有统计学意义(P<0.05)。食管癌患者术后CD-RISC中乐观、自强、坚韧3个维度评分及总分均低于国内常模,差异均有统计学意义(P<0.05)。食管癌患者术后SPBS中情感负担、身体负担、经济负担3个维度评分及总分均高于国内常模,差异均有统计学意义(P<0.05)。食管癌患者术后EORCT QLQ-C30中情感功能、角色功能、认知功能、躯体功能、社会功能5个维度评分及总分均低于国内常模,差异均有统计学意义(P<0.05)。Pearson相关分析结果显示,食管癌患者术后病耻感、自我感受负担评分与生活质量评分呈负相关(P<0.05),心理弹性评分与生活质量评分呈正相关(P<0.05)。多元线性回归分析结果显示,患者术后病耻感、心理弹性、自我感受负担是患者生活质量的影响因素。结论食管癌患者术后心理弹性水平较低,与生活质量呈正相关;术后病耻感与自我感受负担水平较高,与生活质量呈负相关。在临床护理中应重视对患者病耻感与自我感受负担水平的评估,从而采取针对性措施干预,以提升患者心理弹性水平与生活质量。
Objective To investigate the correlation between stigma,psychological resilience,self-perceived burden,and quality of life in patients after esophageal cancer surgery.Methods A total of 100 patients who received esophageal cancer surgery for the first time in Anyang Cancer Hospital from January 2020 to May 2021 were selected as the study participants.The patients were investigated by the Social Impact Scale(SIS),Psychological Resilience Scale(CD-RISC),Self-Perception Burden Scale(SPBS),and Quality of Life Inventory Scale(EORCT QLQ-C30).The correlation between postoperative stigma,psychological resilience,self-perception burden,and quality of life was analyzed using Pearson correlation analysis.Results The subscale scores of economic discrimination,social isolation,internal stigma,social exclusion,and total score in the SIS of patients with esophageal cancer after operation were significantly higher than the Chinese norm(P<0.05).The subscale scores of optimism,self-reliance and tenacity in the CD-RISC and the total score in patients with esophageal cancer after operation were significantly lower than the Chinese norm(P<0.05).The subscale scores of emotional burden,physical burden,economic burden and total score in the SPBS of patients with esophageal cancer after operation were significantly higher than the Chinese norm(P<0.05).The scores of emotional,role,cognitive,physical,and social functioning in the EORCT QLQ-C30 of patients with esophageal cancer after operation were significantly lower than the Chinese norm(P<0.05).The results of Pearson correlation analysis showed that the scores of stigma and self-perceived burden were negatively correlated with the scores of quality of life(P<0.05),and the scores of psychological resilience were positively correlated with the scores of quality of life(P<0.05).Multiple linear regression analysis showed that postoperative stigma,psychological resilience and self-perceived burden were the influencing factors for patients'quality of life.Conclusion The level of postoperative psychological resilience in patients with esophageal cancer remains low,which is positively correlated with the quality of life;the level of postoperative stigma and the self-perceived burden is high,which is negatively correlated with the quality of life.Attention should be paid to the evaluation of the level of stigma and self-perceived burden in patients in clinical nursing,so as to take targeted measures for intervention to improve the level of psychological resilience and quality of life of patients.
