摘要
目的探讨百合乌药泻心汤对脾胃湿热型浅表性胃炎患者临床症状、胃镜病理表现及胃黏膜细胞因子水平的影响。方法选择120例脾胃湿热型浅表性胃炎患者作为研究对象,随机分为观察组与对照组,各60例。对照组予阿莫西林、奥美拉唑、克拉霉素三联疗法进行治疗,观察组在对照组基础上联合百合乌药泻心汤口服,持续用药2周。比较2组临床疗效、中医证候、胃镜黏膜征象、病理学组织表现和胃黏膜细胞因子。结果观察组治疗后总有效率(95.00%,57/60)与对照组(91.67%,55/60)无显著差异(P>0.05)。治疗后2组胃脘疼痛、痞满不适和恶心欲吐中医证候评分均低于治疗前,且观察组低于对照组(P<0.05)。治疗后2组黏膜血管显露、黏膜皱襞变薄和黏膜色泽异常胃镜黏膜征象评分均低于治疗前,且观察组低于对照组(P<0.05);治疗后2组病理学组织表现明显改善,且观察组优于对照组(P<0.05);治疗后2组血清白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)和IL-8水平均低于治疗前,且观察组低于对照组(P<0.05)。结论百合乌药泻心汤可以显著改善脾胃湿热型浅表性胃炎患者胃脘疼痛、痞满不适和恶心欲吐临床症状,减轻胃黏膜组织病变程度,控制炎症水平。
Objective To explore the effects of Baihe Wuyao Xiexin Decoction on clinical symptoms,gastroscopic pathological manifestations and levels of gastric mucosal cytokines in patients with superficial gastritis of spleen-stomach damp-heat type.Methods A total of 120 patients with superficial gastritis of spleen-stomach damp-heat type admitted to the hospital were enrolled as the research objects and were divided into the observation group and the control group randomly,60 cases in each group.The control group was treated with triple therapy(amoxicillin,omeprazole,clarithromycin),while on the basis of the treatment in the control group,the observation group was additionally treated with Baihe Wuyao Xiexin Decoction orally.All were continuously treated for 2 weeks.The clinical curative effect,Traditional Chinese Medicine(TCM)syndromes,gastroscopic mucosal signs,pathological manifestations and gastric mucosal cytokines were compared between the 2 groups.Results After treatment,there was no statistically significant difference in total response rate between the observation group and the control group(95.00%vs91.67%)(P>0.05).After treatment,scores of TCM syndromes(epigastric pain,distention and fullness,nausea and vomiting)in both groups were lower than those before treatment,which were lower in the observation group than the control group(P>0.05).After treatment,scores of gastroscopic mucosal signs(mucosal vessel exposure,thinning of mucosal folds,abnormal mucosal color)in both groups were lower than those before treatment,and the scores in the observation group were lower than those in the control group(P<0.05).After treatment,pathological manifestations in both groups were significantly improved,and the observation group was better than the control group(P<0.05).After treatment,levels of serum interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)and IL-8 in both groups were lower than those before treatment,and the levels in the observation group were lower than those in the control group(P<0.05).Conclusion Baihe Wuyao Xiexin Decoction can significantly improve clinical symptoms(epigastric pain,distention and fullness,nausea and vomiting)in patients with superficial gastritis of spleen-stomach damp-heat type,relieve lesion degree of gastric mucosal tissues and control inflammation level.
