摘要
目的 探讨围手术期肺康复对胸腔镜下肺癌根治患者日常生活能力(ADL)和生活质量的影响。方法选取中南大学湘雅医院心胸外科2019年1—6月胸腔镜下行肺癌根治的94例患者为对照组,接受心胸外科围手术期常规治疗;选取2019年7—12月由同一组医生行肺癌根治的患者100例为试验组,在常规治疗基础上实施呼吸锻炼和循序渐进活动为主的围手术期肺康复训练。于入院时、术后第1天、术后第3天、出院前收集患者ADL(Barthel指数量表)评分,于入院时和出院后一个月收集患者生活质量评估量表(EORTC-QLQ-C30)评分,比较两组患者ADL和生活质量状况。结果 试验组患者术后第1天、术后第3天和出院前ADL评分均高于对照组,差异有统计学意义(P <0.05);出院后1个月除疼痛、失眠外,试验组生活质量其他指标均优于对照组,差异有统计学意义(P <0.05)。结论 围手术期肺康复可以提高胸腔镜下肺癌根治患者的ADL,提高生活质量。
Objective To investigate the effects of perioperative pulmonary rehabilitation on the activities of daily living(ADL)and quality of life(QoL)of patients undergoing thoracoscopic radical resection of lung cancer.Methods A total of 94 patients who underwent thoracoscopic radical resection of lung cancer from January to June 2019 in the Department of Cardiothoracic Surgery of Xiangya Hospital Central South University were selected as the control group and received conventional perioperative treatment of cardiothoracic surgery.Another 100 patients who underwent radical resection of lung cancer from July to December 2019 by the same group of surgeons were selected as the experimental group and received perioperative pulmonary rehabilitation training based on breathing exercises and progressive activities on the basis of conventional treatment.The ADL(Barthel index scale)scores were collected at the time of admission,on the postoperative D 1 and D 3,and before discharge,and scores of the European Organization for research and treatment of cancer-quality of life questionnaire-C30(EORTC-QLQ-C30)were collected at the time of admission and 1 month after discharge to compare the ADL and QoL status between the two groups of patients.Results The ADL scores of patients in the experimental group were higher than those of patients in the control group on the postoperative D 1 and D 3,and before discharge,with statistically significant differences(P<0.05).1 month after discharge,all QoL indicators except for pain and insomnia in the experimental group were significantly better than those in the control group,with statistically significant differences(P<0.05).Conclusion Perioperative pulmonary rehabilitation can improve the ADL and QoL of patients undergoing thoracoscopic radical resection of lung cancer.
