摘要
胫骨平台位于胫骨近端,其上端膨大,向两侧突出,在人身体的膝关节部位的负重中,起到关键性作用。临床数据表明,胫骨平台骨折非常常见,占全身骨折的1.9%。胫骨平台骨折通常伴有内外侧半月板、关节软骨及韧带的严重损伤,在临床治疗过程中,无论是治疗措施不当还是医生漏诊、误诊都能够对膝关节功能形成严重的影响,进而造成膝关节畸形、关节稳定性及下肢力线改变,对患者日后的生活质量造成严重的影响。随着医学技术的不断发展,临床工作人员对于胫骨平台骨折的分型、影像学诊断及治疗方案有了全新的认识,有效促进了骨折愈合后膝关节功能的恢复,降低了创伤性关节炎、关节僵直等相关并发症发生率。在治疗过程中,由于患者的胫骨平台受损机制和分型均不同,因此采取相应的治疗方案也有所不同。本文根据多年的临床数据和经验对胫骨平台骨折治疗的研究进展进行了综述,为胫骨平台骨折的临床治疗提供参考方案。
The tibial plateau is located at the proximal end of the tibia. Its upper end expands and protrudes to both sides. It plays the key role in the weight-bearing of the knee joint of human body. Clinical data show that tibial plateau fractures are very common,accounting for 1.9% of the total body fracture. Tibial plateau fractures are usually accompanied by severe injuries of medial and lateral meniscus,articular cartilage and ligament. In the process of clinical treatment,whether it is improper treatment measures or doctors missed diagnosis,misdiagnosis can form a serious impact on the function of the knee joint,and then cause knee joint deformity,joint stability and lower limb force line change,which has a serious impact on the quality of life of patients in the future. With the continuous development of medical technology,clinical staff have a new understanding of the classification,imaging diagnosis and treatment of tibial plateau fractures,which effectively promotes the recovery of knee joint function after fracture healing,and reduces the incidence of traumatic arthritis,joint stiffness and other related complications. In the process of treatment,due to the different mechanism and classification of tibial plateau damage,the corresponding treatment plan is also different. Based on many years of clinical data and experience,this article reviews the research progress in the treatment of tibial plateau fracture,in order to provide the reference scheme for the clinical treatment of tibial plateau fracture.
