摘要
目的探讨活血止痛汤联合神经肌肉电刺激对胫骨平台骨折术后患者骨代谢及膝关节功能的影响.方法选取2019年7月—2021年4月该院收治的84例胫骨平台骨折术后患者为研究对象,按随机数字表法分为两组,各42例.对照组采用神经肌肉电刺激治疗,观察组在对照组基础上联合活血止痛汤治疗,均持续治疗4周.对比两组治疗前后骨代谢指标、膝关节功能及关节疼痛、肿胀程度.结果治疗前,两组骨代谢指标、膝关节功能、关节疼痛、肿胀程度比较,组间差异无统计学意义(P>0.05).治疗后,试验组基质金属蛋白酶-2、骨形态发生蛋白-2水平及Lysholm膝关节功能评分量表评分分别为(220.23±22.69)ng/L、(684.28±56.10)ng/L、(59.88±6.32)分,均高于对照组的(193.35±21.32)ng/L、(624.35±54.58)ng/L、(52.05±7.41)分,组间差异有统计学意义(P<0.05);试验组疼痛视觉模拟评分法评分与肿胀值分别为(1.47±0.35)分、(2.29±0.61)cm,均优于对照组的(2.39±0.74)分、(3.48±0.87)cm,组间差异有统计学意义(P<0.05).结论胫骨平台骨折术后患者采用活血止痛汤联合神经肌肉电刺激治疗的临床效果确切,可以减轻关节疼痛、肿胀程度,提高骨代谢水平,改善膝关节功能.
Objective To investigate the effect of Huoxue Zhitong decoction combined with neuromuscular electrical stimulation on bone metabolism and knee joint function in patients with tibial plateau fractures after operation.Methods A total of 84 patients with tibial plateau fractures after operation treated in this hospital from July 2019 to April 2021 were selected as the research objects,and divided into two groups according to the random number table method,with 42 cases in each group.The control group was treated with neuromuscular electrical stimulation,and the observation group was treated with Huoxue Zhitong decoction on the basis of the control group,and the treatment continued for 4 weeks.The bone metabolism indexes,knee joint function,and the degree of joint pain and swelling were compared between the two groups before and after treatment.Results Before treatment,there was no significant difference in bone metabolism indexes,knee joint function,joint pain and swelling degree between the two groups(P>0.05).After treatment,the levels of matrix metalloproteinase-2,bone morphogenetic proteins-2 and Lysholm knee score scale in the experimental group were(220.23±22.69)ng/L,(684.28±56.10)ng/L,and(59.88±6.32)points,respectively,which were higher than(193.35±21.32)ng/L,(624.35±54.58)ng/L,(52.05±7.41)points of the control group,and the differences between the groups were statistically significant(P<0.05).The pain visual analog scale scores and swelling value of the experimental group were(1.47±0.35)points and(2.29±0.61)cm,respectively,which were better than(2.39±0.74)points and(3.48±0.87)cm of the control group,respectively,and the differences between the groups were statistically significant(P<0.05).Conclusion The clinical effect of Huoxue Zhitong decoction combined with neuromuscular electrical stimulation in patients after tibial plateau fracture surgery is definite,which can reduce the degree of joint pain and swelling,improve the level of bone metabolism,and improve the function of the knee joint.
