Expression of MCP-1,VCAM-1 and LTB4 in serum of children with hand-foot-mouth disease and their predictive value for prognosis

摘要 目的探讨血清单核细胞趋化蛋白-1(MCP-1)、血管细胞间黏附分子-1(VCAM-1)、白三烯B4(LTB4)在手足口病(HFMD)患儿中的表达及对预后的预测价值。方法选取2018年4月至2021年4月海南医学院第一附属医院收治的HFMD患儿136例为观察组,同期选取60例体检健康儿童为对照组。比较两组血清MCP-1、VCAM-1、LTB4水平,分析HFMD患儿血清MCP-1、VCAM-1、LTB4水平与病情程度相关性,Logistic回归分析重症HFMD患儿并发神经源性肺水肿(NPE)的影响因素,受试者工作特征(ROC)曲线分析血清MCP-1、VCAM-1、LTB4对重症HFMD患儿并发NPE的预测价值。结果观察组血清MCP-1、VCAM-1、LTB4水平高于对照组,差异有统计学意义(P<0.05);HFMD患儿血清MCP-1、VCAM-1、LTB4水平与病情程度呈正相关(P<0.05);Logistic回归分析显示,血清MCP-1、VCAM-1、LTB4高表达均为重症HFMD患儿并发NPE的独立危险因素(P<0.05);ROC曲线分析显示,血清MCP-1、VCAM-1、LTB4联合预测重症HFMD患儿并发NPE的AUC为0.910,高于单独预测(P<0.05)。结论HFMD患儿血清MCP-1、VCAM-1、LTB4水平与病情严重程度呈正相关,且对重症患儿并发NPE具有预警价值,有助于指导临床救治。 Objective To explore the expression of serum monocyte chemoattractant protein-1(MCP-1),vascular cell adhesion molecule-1(VCAM-1),and leukotriene B4(LTB4)in children with hand-foot-mouth disease(HFMD)and prognostic value.Methods From April 2018 to April 2021,136 children with HFMD in the First Affiliated Hospital of Hainan Medical College were selected as the observation group,and 60 healthy children were selected as the control group during the same period.The serum levels of MCP-1,VCAM-1,and LTB4 were compared between the two groups.The correlation between serum MCP-1,VCAM-1,LTB4 levels and disease severity in children with HFMD was analyzed.Logistic regression was used to analyze the influencing factors of neurogenic pulmonary edema(NPE)in children with severe HFMD.The receiver operating characteristic(ROC)curve was used to analyze the predictive value of serum MCP-1,VCAM-1,and LTB4 in children with severe HFMD complicated by NPE.Results The levels of serum MCP-1,VCAM-1 and LTB4 in the observation group were higher than those in the control group,the difference was statistically significant(P<0.05).The levels of serum MCP-1,VCAM-1 and LTB4 in children with HFMD were positively correlated with the severity of the disease(P<0.05).Logistic regression analysis showed that the high expression of serum MCP-1,VCAM-1 and LTB4 were independent risk factors for NPE in children with severe HFMD(P<0.05).ROC curve analysis showed that the combination of serum MCP-1,VCAM-1,and LTB4 predicted the AUC of NPE in children with severe HFMD at 0.910,which was higher than the single prediction(P<0.05).Conclusion Serum MCP-1,VCAM-1,LTB4 levels in children with HFMD are positively correlated with the severity of the disease,and have early warning value for severely ill children with NPE,which can help guide clinical treatment.
