摘要
目的:观察血府逐瘀汤合真武汤联合常规西药治疗冠心病心力衰竭气虚血瘀证患者的效果。方法:选取2019年10月至2021年9月该院收治的80例冠心病心力衰竭气虚血瘀证患者进行前瞻性研究,按照随机数字表法将其分为观察组和对照组各40例。对照组予以常规西药治疗,观察组在对照组基础上予以血府逐瘀汤合真武汤治疗。比较两组临床疗效、治疗前后中医证候积分、心功能指标[左心室舒张末期内径(LVEDD)、左心室收缩末期内径(LVESD)、左心室射血分数(LVEF)]水平、血清N末端脑钠肽前体(NTproBNP)水平,以及不良反应发生率。结果:观察组治疗总有效率为95.00%(38/40),高于对照组的80.00%(32/40),差异有统计学意义(P<0.05);治疗后,观察组中医证候积分低于对照组,差异有统计学意义(P<0.05);观察组LVEDD、LVESD、NT-proBNP水平均低于对照组,LVEF水平高于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:血府逐瘀汤合真武汤治疗冠心病心力衰竭气虚血瘀证患者可提高治疗总有效率,降低中医证候积分和NT-proBNP水平,改善心功能指标水平,效果优于单纯常规西药治疗。
Objective:To observe effects of Xuefu Zhuyu decoction and Zhenwu decoction combined with conventional western medicine in treatment of coronary heart disease and heart failure patients with qi deficiency and blood stasis syndrome.Methods:A prospective study was conducted on 80 coronary heart disease and heart failure patients with qi deficiency and blood stasis syndrome admitted to this hospital from October 2019 to September 2021.They were divided into observation group and control group according to the random number table method,40 cases in each.The control group was treated with conventional western medicine,while the observation group was treated with Xuefu Zhuyu decoction and Zhenwu decoction on the basis of that of the control group.The clinical efficacy,the TCM syndrome scores before and after the treatment,the cardiac function index levels[left ventricular end-diastolic diameter(LVEDD),left ventricular end-systolic diameter(LVESD),left ventricular ejection fraction(LVEF)],the serum N-terminal pro-brain natriuretic peptide(NT-proBNP)level,and the incidence of adverse reactions were compared between the two groups.Results:The total effective rate of treatment in the observation group was 95.00%(38/40),which was higher than 80.00%(32/40)in the control group,and the difference was statistically significant(P<0.05).After the treatment,the TCM syndrome score of the observation group was lower than that of the control group,and the difference was statistically significant(P<0.05).The levels of LVEDD,LVESD and NT-proBNP in the observation group were lower than those in the control group,the level of LVEF was higher than that in the control group,and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions:Xuefu Zhuyu decoction combined with Zhenwu decoction can improve the total effective rate of treatment,reduce TCM syndrome scores and NT-proBNP levels,and improve cardiac function index levels in the coronary heart disease and heart failure patients with qi deficiency and blood stasis syndrome.Moreover,it is superior to single conventional western medicine treatment.
