摘要
目的探讨Kringle结构IV型-2亚型结构(KIV-2)拷贝数及载脂蛋白a[Apo(a)]颗粒大小与2型糖尿病和冠心病的相关性。方法回顾性选取2019年6月至2020年6月于复旦大学附属中山医院青浦分院就诊的180例体检者为研究对象,对临床资料进行收集和分析后将其分为3组:正常对照组、冠心病组和糖尿病组,每组各60例。检测3组人群的空腹血糖、餐后2 h血糖(2 h PG)、糖化血红蛋白(HbA1c)、总胆固醇、甘油三酯、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)和脂蛋白(a)[Lp(a)]含量;通过RT-PCR法测定KIV-2拷贝数,并通过检测KIV-2结构重复数确定Apo(a)颗粒大小;分析Lp(a)拷贝数、Apo(a)颗粒大小分别与2型糖尿病和冠心病的关系。结果冠心病组和糖尿病组空腹血糖、2 h PG、HbA1c、总胆固醇、甘油三酯、LDL-C、Lp(a)水平均高于正常对照组,HDL-C、KIV-2拷贝数低于正常对照组,差异均有统计学意义(P<0.05),而冠心病组与糖尿病组HDL-C水平比较,差异无统计学意义(P>0.05)。2型糖尿病和冠心病分别与KIV-2拷贝数相关性显著,与Apo(a)颗粒大小相关性不显著,且LP(a)浓度与KIV-2拷贝呈负相关。结论Apo(a)颗粒大小与2型糖尿病和冠心病无明显相关性,而KIV-2拷贝数与2型糖尿病和冠心病有明显相关性。检测KIV-2拷贝数可为2型糖尿病和冠心病的诊断与治疗提供依据。
Objective To investigate the correlation of Kringle structure type IV-2 isoform structure(KIV-2)copy number and apolipoprotein a[Apo(a)]particle size with type 2 diabetes and coronary heart disease.Methods A total of 180 physical examinees who visited Qingpu Branch of Zhongshan Hospital,Fudan University from June 2019 to June 2020 were retrospectively selected as research objects.After collecting and analyzing clinical data,they were divided into three groups:normal control group,coronary heart disease group and diabetes group,60 cases in each group.The contents of fasting blood glucose,2 h postprandial glucose(2 h PG),glycosylated hemoglobin(HbA1c),total cholesterol,triglycerides,low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C)and lipoprotein(a)[Lp(a)]in the three groups were measured;the copy number of KIV-2 was determined by RT-PCR,and the particle size of Apo(a)was determined by detecting the repeat number of KIV-2 structure;the relationship between Lp(a)copy number,Apo(a)particle size and type 2 diabetes and coronary heart disease was analyzed.Results The levels of fasting blood glucose,2 h postprandial blood glucose,HbA1c,total cholesterol,triglycerides,LDL-C,Lp(a)in the coronary heart disease group and the diabetes group were higher than those in the normal control group,and the copies of HDL-C and KIV-2 were lower than those in the normal control group,the differences were statistically significant(P<0.05),while the HDL-C levels in the coronary heart disease group and the diabetes group were not significantly different(P>0.05).Type 2 diabetes and coronary heart disease were significantly correlated with KIV-2 copy number,respectively,but not with Apo(a)particle size,and LP(a)concentration was negatively correlated with KIV-2 copy.Conclusion Apo(a)particle size has no significant correlation with type 2 diabetes and coronary heart disease,which provides a basis for clinical diagnosis and treatment.
