摘要
目的探讨闭合复位弹性髓内钉治疗儿童股骨干骨折的临床疗效。方法采用闭合复位弹性髓内钉治疗30例股骨干骨折患儿。记录骨折复位情况、骨折愈合情况、并发症发生情况,采用Flynn评价标准评价临床疗效。结果患儿均获得随访,时间12~32个月。骨折复位满意29例,无旋转成角畸形;1例下肢延长。患儿骨折均愈合良好,时间7~13周。术中均无血管神经损伤;术后无骨折移位、骨不连等并发症发生;1例出现钉尾激惹,取出弹性髓内钉后症状消失。末次随访时采用Flynn评价标准评价临床疗效:优27例,良3例,优良率30/30。结论采用闭合复位弹性髓内钉治疗儿童股骨干骨折具有操作简单、术中透视次数少等优点,且避免了切开复位造成的骨折端血运破坏。
Objective To evaluate the clinical therapeutic effects of closed reduction and elastic intramedullary nail for treatment of femoral shaft fractures in children.Methods The 30 patients with femoral shaft fractures in children were treated with closed reduction and elastic intramedullary nail.The reduction conditions,fracture healing and complications were recorded.At last follow-up,the functional outcomes were assessed with Flynn criteria.Results All patients were all followed up for 12~32 months.The satisfactory fracture reduction was obtained in 29 cases,there was no rotational angular deformity,and 1 case of lower limb lengthening.In 7~13 weeks postoperation,all fractures healed well in the children.There was no complications such as neurovascular injures,and fracture displacement,nonunion;and 1 case had irritation at the nail end,and the symptom disappeared after removal of the intramedullary nail.At the last follow-up,the effect was assessed according to the Flynn evaluation criteria,the result was excellent in 27 cases,good in 3 cases,with an excellent-good rate of 30/30.Conclusions The application of closed reduction and elastic intramedullary nail is a safe and effective method for treatment of the femoral shaft fracture in children,with fewer intraoperative fluoroscopy times,which avoid to damage the blood supply at the fracture end caused by open reduction.
