摘要
目的:观察基于经筋理论应用关刺联合耳穴埋豆法治疗慢性非特异性下腰痛(CNLBP)的临床疗效。方法:将60例CNLBP患者随机分为关刺组30例与常规组30例。关刺组予以关刺联合耳穴埋豆干预,常规组予以常规针刺联合耳穴埋豆干预。两组疗程均为4周,疗程结束后2个月进行随访。其中,关刺组隔1 d治疗1次,4周内共治疗14次。常规组每天治疗1次,每周治疗6 d,休息1 d,4周内共治疗24次。采用疼痛视觉模拟VAS评分、功能障碍指数ODI评分和日本骨科协会腰椎JOA评分进行比较,观察第1次治疗后的即时镇痛效果、疗程结束后及随访的临床疗效。结果:两组第1次治疗后与治疗前比较,VAS评分均显著下降,差异具有统计学意义(P<0.01),且关刺组优于常规组;治疗4周后及治疗结束后2个月随访的ODI评分均明显下降,差异具有统计学意义(P<0.01),JOA评分均显著上升,差异具有统计学意义(P<0.01),且关刺组优于常规组;经4周治疗后,关刺组总有效率93%(28/30),常规组总有效率83%(25/30),关刺组与常规组疗效相当,差异无统计学意义(P>0.05);疗程结束后2月随访,关刺组总有效率为93%(28/30),常规组总有效率为77%(23/30),关刺组疗效优于常规组,差异具有统计学意义(P<0.05)。结论:关刺法与常规针刺法治疗慢性非特异性下腰痛均有较好疗效,关刺法优于常规针刺法,另外在经筋理论指导下关刺联合耳穴埋豆法治疗CNLBP在即刻镇痛和远期疗效方面较常规针刺联合耳穴埋豆疗效更加显著,且能够减少针刺次数。
Objective:To observe the clinical efficacy of joint needling combined with auricular acupressure with herbal seeds in treatment of chronic non-specific low back pain(CNLBP)based on meridian sinew theory.Methods:60 patients with CNLBP were randomly divided into the study group(n=30)and the control group(n=30).The study group was treated with joint needling(once every other day,14 times in total)and auricular acupressure with herbal seeds,and the control group was treated with routine acupuncture(once every day,24 times in total)and auricular acupressure with herbal seeds.The treatment course for both groups was 4 weeks,and the patients were followed up 2 months after the treatment.The scores of VAS,ODI and JOA were compared to observe the immediate analgesic effect after the first treatment,clinical efficacy after the treatment and the effect during the follow-up.Results:VAS score decreased significantly after the first treatment compared with that before the treatment in both groups(P<0.01),and the decrease in the study group was more significant than that in the control group.ODI score decreased significantly(P<0.01),and JOA score increased significantly(P<0.01)after 4 weeks of the treatment and during the follow-up;of which the improvements in the study group were more significant than those in the control group.The total effective rate was 93%(28/30)in the study group,and it was 83%(25/30)in the control group after four weeks of the treatment;there was no statistical difference in the total effective rate between the two groups(P>0.05).The total effective rate was 93%(28/30)in the study group,which was significantly higher than 77%(23/30)in the control group during the follow-up(P<0.05).Conclusion:Joint needling and routine acupuncture are both effective for CNLBP,and the joint needling is better than routine acupuncture.The curative effect of joint needling and auricular acupressure with herbal seeds based on meridian sinew theory is more significant than that of routine acupuncture in treatment of CNLBP in terms of immediate analgesia and long-term curative effect,and it can reduce the number of acupuncture.
