摘要
目的探讨补肾通络方结合经皮穴位电刺激应用于肝肾亏虚型膝关节骨性关节炎(KOA)康复治疗中的效果.方法选取阳信县中医医院2020年8月—2022年9月收治的88例肝肾亏虚型KOA患者为研究对象,通过随机抽签法将其分为对照组及观察组.对照组(n=44)采用补肾通络方治疗,观察组(n=44)在此基础上采用经皮穴位电刺激治疗,均持续30 d.比较两组患者的中医证候积分、膝关节功能、疼痛情况及生活质量.结果治疗后,观察组的关节疼痛、肿胀、畸形、屈伸不利、头晕、耳鸣中医证候积分均低于对照组,组间差异有统计学意义(P<0.05).治疗后,观察组的西安大略和麦克马斯特大学骨关节炎指数评分均低于对照组,Lysholm膝关节功能评分高于对照组,组间差异有统计学意义(P<0.05);观察组的疼痛视觉模拟评分为(2.16±0.32)分,低于对照组的(4.55±0.68)分,差异有统计学意义(P<0.05);观察组的C反应蛋白、白细胞介素-6、白细胞介素-1β水平均低于对照组,组间差异有统计学意义(P<0.05);观察组的世界卫生组织生活质量评定量表简表中各维度评分均高于对照组,组间差异有统计学意义(P<0.05).结论肝肾亏虚型KOA患者采用补肾通络方结合经皮穴位电刺激治疗的效果显著,有利于减轻其临床症状、机体炎症及疼痛水平,并可改善其膝关节功能,提高生活质量,值得临床推广使用.
Objective To investigate the effect of Bushen Tongluo Prescription combined with percutaneous acupoint electrical stimulation on the rehabilitation of knee osteoarthritis(KOA)of liver and kidney deficiency.Methods 88 KOA patients of liver and kidney deficiency type admitted to Yangxin County Hospital of Traditional Chinese Medicine from August 2020 to September 2022 were selected as the study objects and were divided into a control group and an observation group by random drawing method.The control group(n=44)were treated with Bushen Tongluo Prescription,the observation group(n=44)were treated with percutaneous acupoint electrical stimulation on this basis.Both groups were treated for 30 days.The TCM syndrome scores,knee joint function,pain and quality of life were compared between the two groups.Results After treatment,the scores of TCM syndrome such as joint pain,swelling,deformity,adverse flexion and extension,dizziness and tinnitus in the observation group were lower than those in the control group,and the differences between the groups were statistically significant(P<0.05);the Western Ontario and McMaster Universities osteoarthritis index score in the observation group was lower than that in the control group,and the Lysholm knee function score was higher than that in the control group,and the differences between the groups were statistically significant(P<0.05);the pain Visual Analogue Scale score of the observation group was(2.16±0.32)points,which was lower than(4.55±0.68)points of the control group,and the difference was statistically significant(P<0.05);the levels of C-reactive protein,interleukin-6 and interleukin-1βin the observation group were lower than those in the control group,and the differences between the groups were statistically significant(P<0.05);the scores of all dimensions of the World Health Organization quality of life-BREF scale in the observation group were higher than those in the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion Bushen Tongluo Prescription combined with percutaneous acupoint electrical stimulation can significantly reduce the clinical symptoms,inflammation and pain of KOA patients with liver and kidney deficiency,and can improve the function of the knee joint and improve the quality of life,which is worthy of clinical promotion.
