摘要
目的探讨小剂量舒芬太尼复合罗哌卡因用于妊娠期糖尿病产妇剖宫产术中的麻醉效果及对其血糖水平的影响。方法选取2017年1月—2019年1月莆田市涵江区中医院收治的60例妊娠期糖尿病产妇作为研究对象。根据产妇建档编号进行分组,单数为参照组,实施罗哌卡因注射麻醉;双数为研究组,实施小剂量舒芬太尼复合罗哌卡因注射麻醉,每组30例。对比两组产妇麻醉效果、血糖水平及新生儿Apgar评分。结果研究组产妇麻醉起效时间、达到最高阻滞平面时间及运动恢复时间均短于参照组,且研究组产妇麻醉维持时间长于参照组,差异有统计学意义(P<0.05);麻醉后10 min,研究组产妇空腹血糖水平明显低于参照组,差异有统计学意义(P<0.05);同时,研究组新生儿出生后1 min及出生后10 min Apgar评分均高于参照组,差异有统计学意义(P<0.05)。结论在妊娠期糖尿病产妇剖宫产术中实施小剂量舒芬太尼复合罗哌卡因进行麻醉,麻醉效果较好,能改善新生儿出生后状态,且能够稳定产妇血糖水平。
Objective To investigate the anesthetic effect of low dose sufentanil combined with ropivacaine in cesarean section of pregnant women with gestational diabetes and its influence on blood glucose level.Methods From January 2017 to January 2019,60 pregnant women with diabetes in Putian Hanjiang District Hospital of Traditional Chinese Medicine were selected as the research objects.The parturients were divided into groups according to the filing number of the parturients.The odd number was the reference group,and ropivacaine injection anesthesia was performed;the even number was the study group,which was anesthetized with small dose sufentanil combined with ropivacaine by injection,with 30 cases in each group.The anesthetic effect,blood glucose level of parturients and Apgar score of newborn were compared between the two groups.Results The onset time of anesthesia,the time to reach the highest block plane and the time of movement recovery in the study group were shorter than those in the reference group,and the maintenance time of anesthesia in the study group was longer than that in the reference group,the difference was statistically significant(P<0.05).10 min after anesthesia,the fasting blood glucose level in the study group was significantly lower than that in the reference group,the difference was statistically significant(P<0.05).At the same time,the Apgar score 1 min and 10 min after birth in the study group was higher than that in the reference group,the difference was statistically significant(P<0.05).Conclusion In cesarean section of pregnant women with diabetes,a small dose of sufentanil combined with ropivacaine is used for anesthesia.The anesthesia effect is good,it can improve the postnatal state of the newborn,and it can stabilize the maternal blood sugar level.
