摘要
目的分析血浆YKL-40水平与脑出血患者神经功能损害的关系。方法选取2018年1月至2021年12月郑州大学第一附属医院收治的发病3 d内脑出血患者,治疗前采集患者血标本,通过酶联免疫吸附法检测血浆YKL-40水平,使用美国国立卫生研究院卒中量表(NIHSS)评估患者神经功能。结果对426例有完整数据的第1次发病的脑出血患者的临床资料进行分析。根据患者血浆YKL-40水平分为低、中及高浓度组。3组年龄、性别、吸烟史、饮酒史、血压比较,差异有统计学意义(P<0.05),其他一般资料比较,差异无统计学意义(P>0.05)。3组NIHSS评分的分布存在统计学差异(χ^(2)=22.189,P=0.015)。低、中及高浓度组患者入院时NIHSS评分≥8分的患者分别为13、26、37例,对应占比分别为9.15%、18.31%、26.06%。校正混杂因素后中、高浓度组患者入院时NIHSS≥8分的占比仍高于低浓度组,差异有统计学意义(P<0.05)。年龄在60岁以下、60岁及以上的患者中,与低浓度组患者相比,高浓度组患者入院时NIHSS评分≥8分的占比增高,差异有统计学意义(P=0.044、0.034);高浓度组男性和女性入院时NIHSS评分≥8分的占比虽然有所升高,但与低浓度组比较差异无统计学意义(P=0.128、0.160)。结论脑出血患者的血浆YKL-40水平与神经功能损害程度呈正相关。YKL-40可作为筛选重型脑出血患者的血液标志物。
Objective To analyze the relationship between plasma YKL-40 level and neurological impairment in patients with cerebral hemorrhage.Methods Patients with cerebral hemorrhage within 3 days of onset were selected from January 2018 to December 2021 in the First Affiliated Hospital of Zhengzhou University,and blood samples were collected before treatment after admission.The plasma YKL-40 level of patients was detected by enzyme-linked immunosorbent assay,and the neurological function was evaluated by the National Institutes of Health stroke scale(NIHSS).Results The clinical data of 426 patients with cerebral hemorrhage in the first episode with complete data were analyzed and they were divided into low,medium and high concentration groups according to their plasma YKL-40 level.There were statistical differences in age,gender,smoking history,drinking history and blood pressure among the three groups(P<0.05),while there were no statistical difference in other general data(P>0.05).The distribution of NIHSS scores in the three groups was statistically different(χ^(2)=22.189,P=0.015).There were 13,26 and 37 patients with NIHSS score≥8 in the low,medium and high concentration groups,respectively,accounting for 9.15%,18.31%and 26.06%.After adjusting for confounding factors,the proportion of patients with NIHSS score≥8 in the middle and high concentration groups was still higher than that in the low concentration group at admission,with a statistical difference(P<0.05).Among the patients under 60 years old,60 years old and above,the proportion of NIHSS score≥8 points in the high concentration group was higher than that in the low concentration group at admission,the difference was statistically significant(P=0.044,0.034).Although the proportion of NIHSS score≥8 points in male and female in high concentration group was increased,there was no statistical difference compared with low concentration group(P=0.128,0.160).Conclusion The level of plasma YKL-40 in patients with cerebral hemorrhage was positively correlated with the degree of neurological impairment.YKL-40 can be used as a blood marker for screening patients with severe cerebral hemorrhage.
