摘要
目的探讨自体骨移植与富血小板血浆联合体外冲击波治疗胫骨远端骨不连的临床效果。方法对2019年2月-2020年2月收治的50例胫骨远端骨不连患者按随机数字表法分为对照组和观察组,每组各25例,均行体外冲击波治疗。其中对照组予以自体骨移植治疗,观察组予以自体骨移植与富血小板血浆治疗,观察两组治疗效果、治疗后愈合时间、骨痂X线评分及不良反应发生情况。结果观察组总有效率为92.00%(23/25),明显高于对照组64.00%(16/25)(P<0.05);观察组临床愈合时间及骨性愈合时间均短于对照组(P<0.05);两组的骨痂X线评分均上升,观察组高于对照组(P<0.05);观察组不良反应发生率为8.00%(2/25),低于对照组20.00%(5/25)(P>0.05)。结论胫骨远端骨不连患者通过自体骨移植与富血小板血浆联合体外冲击波治疗可促进骨痂的生成,提高临床疗效,缩短愈合时间,且不会增加不良反应。
Objective To investigate the effect of autologous bone transplantation and platelet rich plasma combined with extracorporeal shock wave on the treatment of distal tibial nonunion.Methods According to the random number table method,50 patients with distal tibial nonunion treated in our hospital from February 2019 to February 2020 were divided into control group and observation group.25 cases in each group were treated with extracorporeal shock wave.The control group was treated with autologous bone transplantation,and the observation group was treated with autologous bone transplantation and platelet rich plasma.The therapeutic effects,healing time after treatment,and X-ray score of callus and occurrence of adverse reactions were observed.Results After treatment,the total effective rate in the observation group was 92.00%(23/25),which was significantly higher than 64.00%(16/25)in the control group(P<0.05);the clinical healing time and bone healing time in the observation group were shorter than those in the control group(P<0.05);the X-ray scores of callus in the observation group were higher than those in the control group(P<0.05);the incidence of adverse reactions in the observation group was 8.00%(2/25),which was lower than 20.00%(5/25)in the control group(P>0.05).Conclusion The treatment of distal tibial nonunion with autologous bone transplantation and platelet rich plasma combined with extracorporeal shock wave can promote the formation of callus,improve the clinical curative effect,shorten the healing time and will not increase the adverse reactions.
