核心稳定性训练在痉挛型脑性瘫痪患儿康复中的应用效果及其对血清BDNF及IGF-1水平的影响 被引量:5

Application of core stability training in rehabilitation of children with spastic cerebral palsy and its effect on serum BDNF and IGF-1 levels

摘要 目的研究痉挛型脑性瘫痪(简称脑瘫)患儿应用核心稳定性训练对粗大运动功能、内收肌、腓肠肌表面肌电信号及血清脑源性神经营养因子(BDNF)、胰岛素样生长因子(IGF-1)水平的影响。方法选取2020年10月至2022年2月本院收治住院的痉挛型脑瘫患儿95例作为研究对象,对照组47例给予常规康复,观察组48例联合核心稳定性训练,对比两组患儿粗大运动功能测试量表-88(GMFM-88)各区评分,内收肌、腓肠肌的表面肌电信号均方根值(RMS),神经相关因子水平(BDNF、IGF-1)。结果观察组干预后GMFM-88 D区评分、E区评分均高于对照组,内收肌RMS、腓肠肌RMS水平均低于对照组,BDNF、IGF-1水平均高于对照组,差异有统计学意义(P<0.05)。结论核心稳定性训练对痉挛型脑瘫患儿效果确切,能够有效提升患儿神经相关因子水平,降低肌张力,提高粗大运动功能,具有临床推广价值。 Objective To investigate the effects of core stability training on gross motor function,surface EMG of adductor muscle and gastrocnemius muscle,and serum brain-derived neurotrophic factor(BDNF)and insulin-like growth factor(IGF-1)in children with spastic cerebral palsy(CP for short).Methods A total of 95 children with spastic cerebral palsy admitted to our hospital from October 2020 to February 2022 were selected as research subjects:47 cases in the control group were given routine rehabilitation,and 48 cases in the observation group were additionally given core stability training.The scores of each area of the Gross Motor Function Test Scale 88(GMFM-88)were compared between the two groups.The surface electromyography(RMS)of adductor muscle and gastrocnemius muscle,and the level of nerve related factors(BDNF,IGF-1)were also compared.Results After intervention,the GMFM-88 D zone score and E zone score in the observation group were higher than those in the control group,the levels of adductor muscle RMS and gastrocnemius muscle RMS were lower than those in the control group,and the levels of BDNF and IGF-1 were higher than those in the control group,with statistical difference(P<0.05).Conclusion Core stability training has definite effect on children with spastic cerebral palsy,which can effectively improve the level of nerve-related factors,reduce muscle tension and improve gross motor function in children,and has clinical promotion value.
