摘要
目的探讨重症医学科(ICU)病原菌的分布情况及耐药性,为临床经验治疗和合理用药提供依据。方法运用Excel 2007和Whonet 5.6软件对2020年1月至2022年3月分离自阳江市人民医院ICU的病原菌数据进行统计分析。结果共分离622株非重复病原菌,革兰氏阴性菌、革兰氏阳性菌与真菌分别占56.3%、24.6%、19.1%。主要来源于血液(40.5%)、痰液(23.6%)和尿液(20.9%)。血液的主要分离株是肺炎克雷伯菌和大肠埃希菌。尿液的主要分离株是白色念珠菌。碳青霉烯类耐药肺炎克雷伯菌、碳青霉烯类耐药大肠埃希菌、碳青霉烯类耐药铜绿假单胞菌、碳青霉烯类耐药鲍曼不动杆菌和耐甲氧西林金黄色葡萄球菌的分离率分别是31.5%、3.7%、30.4%、93.3%和54.0%。结论阳江市人民医院ICU病原菌以革兰氏阴性菌为主,主要来源于血液。不同的标本来源,其病原菌构成有所差异。多重耐药菌分离率较高,鲍曼不动杆菌耐药严重。应加强ICU病原菌的耐药性监测、加强抗菌药物管理和医院感染防控力度,促进合理用药,遏制耐药蔓延。
Objective To investigate the distribution and drug resistance of pathogens in intensive care unit (ICU), and to provide basis for clinical treatment and rational drug use. Methods Excel 2007 and Whonet 5.6 software were used to analyze the data of pathogenic bacteria isolated from ICU of Yangjiang People's Hospital from January 2020 to March 2022. Results A total of 622 non-repeating pathogens were isolated, accounting for 56.3% of gram-negative bacteria, 24.6% of gram-positive bacteria and 19.1% of fungi, respectively. The main sources were blood (40.5%), sputum (23.6%) and urine (20.9%). The main isolates of blood were Klebsiella pneumoniae and Escherichia coli. The main isolate of urine is Candida albicans. The isolation rates of carbapenem-resistant Klebsiella pneumoniae, carbapenem-resistant Escherichia coli, carbapenem-resistant Pseudomonas aeruginosa, carbapenem-resistant Acinetobacter baumannii and methicillin-resistant Staphylococcus aureus were 31.5%, 3.7%, 30.4%, 93.3% and 54.0%, respectively. Conclusion The pathogenic bacteria in ICU of Yangjiang People's Hospital were mainly gram-negative bacteria, mainly from blood. The composition of pathogenic bacteria was different in different sample sources. The isolation rate of multidrug-resistant bacteria was high, and the drug resistance of Acinetobacter baumannii was serious. It is necessary to strengthen the monitoring of drug resistance of pathogens in ICU, strengthen the management of antibiotics and the prevention and control of nosocomial infection, promote rational drug use and curb the spread of drug resistance.
