T2DM患者血清UA、FINS、CP、甲状腺激素水平变化及其与血液流变学的相关性 被引量:2

Changes of serum uric acid,fasting insulin,C-peptide,and thyroid hormone levels in patients with T2DM and their correlation with hemorheology

摘要 目的探讨2型糖尿病(T2DM)患者血清尿酸(UA)、空腹胰岛素(FINS)、C肽(CP)、甲状腺激素水平变化及其与血液流变学的相关性。方法选择2021年4月至2022年4月陕西省中医医院收治的90例T2DM患者作为观察组,并选择同期在我院接受体检的90例健康人员作为对照组。比较两组受检者的血清UA、FINS、CP水平、甲状腺激素[促甲状腺素(TSH)、游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)]及血液流变学指标(全血黏度200、全血黏度30、全血黏度5和全血黏度1),比较观察组患者中血液黏度升高及正常患者的血清UA、FINS、CP、TSH、FT3、FT4水平,并采用Pearson相关性分析血清UA、FINS、CP、TSH、FT3、FT4和血液流变学的相关性。结果观察组患者血清UA、FINS、CP、TSH水平为(330.23±27.64)μmol/L、(13.76±2.03)mU/L、(1.69±0.21)μg/L、(3.27±0.36)μmol/L,明显高于对照组的(219.23±32.71)μmol/L、(8.02±1.65)mU/L、(1.20±0.19)μg/L、(1.49±0.23)μmol/L,FT3为(4.18±0.52)pmol/L,明显低于对照组的(4.63±0.69)pmol/L,差异均有统计学意义(P<0.05);观察组患者的全血黏度200、全血黏度30、全血黏度5、全血黏度1分别为(4.23±0.63)mPa·s、(5.39±0.73)mPa·s、(8.31±1.09)mPa·s、(16.39±1.61)mPa·s,明显高于对照组的(3.01±0.46)mPa·s、(4.17±0.69)mPa·s、(6.28±1.17)mPa·s、(12.81±1.65)mPa·s,差异均有统计学意义(P<0.05);观察组患者中血液黏度升高患者的血清UA、FINS、CP、TSH水平分别为(359.27±36.02)μmol/L、(14.90±2.13)mU/L、(1.86±0.27)μg/L、(3.83±0.42)μmol/L,明显高于血液黏度正常患者的(304.33±21.48)μmol/L、(11.49±1.62)mU/L、(1.40±0.21)μg/L、(2.75±0.37)μmol/L,FT3为(3.82±0.54)pmol/L,明显低于血液黏度正常患者的(4.37±0.60)pmol/L,差异均有统计学意义(P<0.05);经Pearson相关性分析结果显示,血清UA、FINS、CP、TSH与全血黏度200、全血黏度30、全血黏度5、全血黏度1均呈正相关(P<0.05),FT3与全血黏度200、全血黏度30、全血黏度5、全血黏度1均呈负相关(P<0.05)。结论T2DM患者血清UA、FINS、CP、TSH水平明显升高,FT3降低,且和血液流变学存在明显相关性。 Objective To study the changes of serum uric acid(UA),fasting insulin(FINS),C-peptide(CP),and thyroid hormone levels in patients with type 2 diabetes mellitus(T2DM)and their correlation with hemorheology.Methods A total of 90 patients with T2DM admitted to Shaanxi Hospital of Traditional Chinese Medicine from April 2021 to April 2022 were selected as the observation group,and 90 healthy people who received physical examination in the hospital during the same period were selected as the control group.The serum UA,FINS,CP levels,thyroid hormones[thyrotropin(TSH),free triiodothyronine(FT3),free thyroxine(FT4)],and hemorheology indexes(whole blood viscosity 200,whole blood viscosity 30,whole blood viscosity 5,and whole blood viscosity 1)were compared between the two groups.In the observation group,the serum UA,FINS,CP,TSH,FT3,and FT4 levels were compared between patients with increased blood viscosity and those with normal blood viscosity.The correlation between serum UA,FINS,CP,TSH,FT3,FT4 and hemorheology was analyzed by Pearson correlation.Results The serum UA,FINS,CP,and TSH levels in the observation group were(330.23±27.64)μmol/L,(13.76±2.03)mU/L,(1.69±0.21)μg/L,(3.27±0.36)μmol/L,which were significantly higher than(219.23±32.71)μmol/L,(8.02±1.65)mU/L,(1.20±0.19)μg/L,(1.49±0.23)μmol/L in the control group;FT3 was(4.18±0.52)pmol/L,which was significantly lower than(4.63±0.69)pmol/L in the control group;the differences were statistically significant(P<0.05).The whole blood viscosity 200,whole blood viscosity 30,whole blood viscosity 5,and whole blood viscosity 1 in the observation group were(4.23±0.63)mPa·s,(5.39±0.73)mPa·s,(8.31±1.09)mPa·s,(16.39±1.61)mPa·s,which were significantly higher than(3.01±0.46)mPa·s,(4.17±0.69)mPa·s,(6.28±1.17)mPa·s,(12.81±1.65)mPa·s in the control group(P<0.05).Serum UA,FINS,CP,and TSH levels in patients with increased blood viscosity in the observation group were(359.27±36.02)μmol/L,(14.90±2.13)mU/L,(1.86±0.27)μg/L,(3.83±0.42)μmol/L,which were significantly higher than(304.33±21.48)μmol/L,(11.49±1.62)mU/L,(1.40±0.21)μg/L,(2.75±0.37)μmol/L in the patients with normal blood viscosity;FT3 was(3.82±0.54)pmol/L,which were significantly lower than(4.37±0.60)pmol/L in the patients with normal blood viscosity;the differences were statistically significant(P<0.05).Pearson correlation analysis showed that serum UA,FINS,CP,TSH were positively correlated with whole blood viscosity 200,whole blood viscosity 30,whole blood viscosity 5,and whole blood viscosity 1(P<0.05),while FT3 was negatively correlated with whole blood viscosity 200,whole blood viscosity 30,whole blood viscosity 5,and whole blood viscosity 1(P<0.05).Conclusion Serum UA,FINS,CP,and TSH levels in T2DM patients are significantly increased,while FT3 is decreased,which all had a significant correlation with hemorheology.
