依达拉奉右莰醇联合阿司匹林治疗急性脑梗死的临床疗效及其对神经功能和氧化应激反应的影响 被引量:8

Clinical efficacy of edaravone and dexborneol combined with aspirin in the treatment of acute cerebral infarction and its effect on neurological function and oxidative stress response

摘要 目的 观察依达拉奉右莰醇联合阿司匹林治疗急性脑梗死的疗效,并评估其对神经功能、氧化应激指标影响。方法 选取龙岩市第二医院于2021年期间收治的确诊为急性脑梗死患者48例为研究对象,依据随机数字表法进行分组分析,分为对照组、研究组,每组例数为24例。两组患者均行常规治疗,对照组予以阿司匹林肠溶片,研究组在对照组基础上加用依达拉奉右莰醇注射用浓溶液,两组均连续治疗14 d。比较两组临床疗效,治疗前后美国国立卫生研究院卒中量表(NIHSS)评分、改良Rankin量表(mRS)评分、脑血流动力学指标[最小血流量(Qmin)、最小血流速度(Vmin)]、血清氧化应激反应指标[丙二醛(MDA)、超氧化物歧化酶(SOD)、去甲肾上腺素(NE)]、炎性反应指标[C反应蛋白(CRP)、基质金属蛋白酶-12(MMP-12)、基质金属蛋白酶-9(MMP-9)],并观察两组不良反应发生情况。结果 研究组治疗总有效率为95.83%,高于对照组的62.50%(χ^(2)=8.084,P=0.004)。治疗14 d后,两组NIHSS评分下降,Qmin、Vmin明显加快,治疗后两组NIHSS评分、mRS评分、Qmin、Vmin比较,差异明显(P<0.01)。治疗14 d后,两组MDA、NE、CRP、MMP-12、MMP-9水平低于治疗前,SOD水平高于治疗前,且研究组MDA、NE、CRP、MMP-12、MMP-9水平水平低于对照组,SOD水平高于对照组(P<0.01)。两组不良反应总发生率统计学对比无差异(P>0.05)。结论 依达拉奉右莰醇结合阿司匹林对于治疗急性脑梗死患者的疗效确切可靠,同时可促进神经功能、脑部血流改善,缓解氧化应激反应,并可改善脑部血流动力学,减轻炎性反应,且安全性较高。 Objective To observe the curative effect of edaravone and dexborneol combined with aspirin in the treat-ment of acute cerebral infarction,and to evaluate the effect of treatment on neurological function and oxidative stress indexes.Methods 48 patients with confirmed acute cerebral infarction admitted to the Second Hospital of Longyan City during2021 were included in the study.They were grouped and analyzed according to the random number table method:the number of patients in the control group and the study group were 24 cases,respectively.The patients in the two groups were treated with routine treatment,the control group was given aspirin enteric coated tablets,the study group was added with edaravone and dexborneol concentrated solution on the basis of the control group,and the two groups were treated continuously for14 days.The clinical effect of the two groups were compared.Before and after treatment,the National Institutes of Health Stroke Scale(NIHSS) score,the modified Rankin Scale(mRS) score,cerebral hemodynamic indicators[minimum blood flow(Qmin),minimum blood flow velocity (Vmin)],serum oxidative stress response indicators[malondialdehyde (MDA),superoxide dismutase (SOD),norepinephrine (NE)],inflammatory response indicators[Creactive protein (CRP),matrix metalloproteinase-12(MMP-12),matrix metalloproteinase-9(MMP-9)],and the incidence of adverse reactions of the two groups were observed.Results The total effective rate of the study group was 95.83%,higher than 62.50%of the control group(χ^(2)=8.084,P=0.004).After 14 days of treatment,the NIHSS score of the two groups was decreased,and Qmin and Vmin were significantly accelerated.There were significant differences in NIHSS score,mRS score,Qmin and Vmin between the two groups after treatment(P<0.01).After 14 days of treatment,the levels of MDA,NE,CRP,MMP-12 and MMP-9 in the two groups were lower than those before treatment,and the level of SOD in the study group was higher than that before treatment.The levels of MDA,NE,CRP,MMP-12 and MMP-9 in the study group were lower than those in the control group,and the level of SOD was higher than that in the control group(P<0.01).The total incidence of adverse reactions in the two groups was closed to statistics,and there was no difference(P>0.05).Conclusion Edaravone and dexborneol combined with aspirin are effective and reliable in improving patients with acute cerebral infarction.At the same time,it can promote neurological function,improve cerebral blood flow,alleviate oxidative stress reaction,improve cerebral blood flow,reduce inflammatory reaction,and has high safety.
