摘要
大气下陷是病毒性心肌炎的主要病机。正气虚弱,邪毒侵犯心脉。随着疾病发展,正邪交争,心体受损,邪毒之气进一步侵损人体正气,气虚下陷,无以行血。本病证候复杂,其病机并非单纯心肺气陷,阴伤、血瘀、痰湿、阳虚都是不可忽视的病理因素。故临床治疗时多以益气升陷为主,佐以养阴、活血、化湿、健脾、温肾等法,以升陷汤为基础,辨证加减,方可提高疗效。若兼血瘀者多加桃仁、红花、川芎、赤芍等以活血通滞、调畅气机;兼痰湿者,可选茯苓、白术等以健脾化痰、理气和中;兼伤阴者,宜以麦冬、生百合等滋润之品以滋阴降火;兼脾虚者,加用党参、白术等益气健脾、补气生血。
Grand Qi sinking is the main pathogenesis of viral myocarditis.When the Zheng Qi is weak,the evil factor would invade the Heart.With the development of the disease,the Zheng Qi and the evil factor are in conflict,during which the mind and body are damaged.When the evil factor Qi further damages the Zheng Qi of the human body,the Qi is deficient and sinks,which is unable to sustain blood circulation.The syndrome of this disease is complex,and its pathogenesis is not simply Heart and Lung Qi sinking.Yin injury,blood stasis,Phlegm Dampness,and Yang deficiency are all evil factors that cannot be ignored.Therefore,in clinical treatment,the main focus is to invigorate Qi to promote the sinking Qi,with methods such as nourishing Yin,promoting blood circulation,removing Dampness,strengthening Spleen,and warming Kidney.On the basis of Shengxian Decoction or modified Shengxian Decoction based on syndrome differentiation can be used to improve the curative effect.For those with blood stasis,Taoren(Semen Persicae),Honghua(Flos Carthami),Chuanxiong(Rhizoma Ligustici Chuanxiong),Chishao(Radix Paeoniae Rubra),etc.can be used to promote blood circulation and relieve stagnation,and regulate Qi movement.For those with Phlegm Dampness syndrome,Fuling(Poria)and Baizhu(Rhizoma Atractylodis Macrocephalae)can be used to invigorate the Spleen and resolve Phlegm,regulate Qi,and neutralize the Qi in Middle Jiao.For those with injured Yin,it is advisable to use moisturizing products such as Maidong(Radix Ophiopogonis)and Shengbaihe(Raw Bulbus Lilii)to nourish Yin and reduce Fire.For those with Spleen deficiency,Dangshen(Radix Codonopsis),Baizhu,etc.can be added to Nourish Qi and invigorate the Spleen,nourish Qi and generate Blood.
